Show moreChildhood obesity is a global health concern that puts children at risk for developing serious health complications. With increasing rates worldwide, it is important to determine how to decrease its prevalence and promote prevention in future generations. Emerging evidence indicating that pre-pregnancy weight status and/or gestational weight gain (GWG) may be linked with overweight/obesity in children. Much of this body of research focused on weight status of offspring at birth and at preschool age. The purpose of this study is to: (1) analyze the research findings regarding obesity in children 5 to 18 years and their mother's pre-pregnancy weight status and/or GWG, and (2) make recommendations for prevention based on a review of current research. A database search of CINAHL, Medline, ERIC and PsycInfo was conducted. A total of 14 articles were identified based on their relevance to key search terms and meeting criteria. This literature review indicated support for associations between an underweight/overweight/obese pre-pregnancy weight status combined with greater than recommended total GWG and higher overweight/obesity in older children and adolescent offspring. Findings also supported the associations between pre-pregnancy weight status with high GWG during early pregnancy and increased offspring overweight/obesity. Pre-pregnancy overweight/obese weight status of mothers was the single factor consistently found to be strongly associated with risk for overweight and obesity in children 5 to 18 years of age. Results of this review support the need for further education, interventions, and policies aimed at healthy nutrition for women during and prior to pregnancy to prevent childhood obesity.

Obese children and adolescents are at increased risk for multiple preventable comorbidities that affect their physical health and psychological well-being. The perception exists that the immigrant child population may possess protection against CO, but this group also experienced an increased prevalence of CO. Acculturation, or the adaptation to American culture and lifestyles, has been shown to be associated with the increased obesity rate among immigrant children and adolescents. Understanding the association between acculturation factors and CO is necessary to effectively combat obesity among children and adolescents in immigrant populations. The purpose of this literature review is to analyze the association of acculturation factors on the prevalence of CO among U.S. immigrant children and adolescents. Only articles that examined CO and at least one aspect of acculturation were included in this review. Twelve articles meeting the criteria were retrieved from CINAHL, Medline, PsychInfo, Academic Search Premier, and ERIC. The review identified the following acculturation factors to be associated with CO: parental immigration factors, child immigration factors, country of birth, generational status and primary home language, parental level of acculturation, and CO related lifestyle practices. There is modest evidence to suggest that a combination of acculturation factors is associated with increasing CO rate in immigrant populations. This review enhances the knowledge and awareness of public and pediatric healthcare professionals to develop targeted interventions that promote healthy weight in immigrant children and adolescents at increased risk for obesity.

This intimate touch helps to replicate aspects of the environment that the preterm infant experienced in utero. The purpose of this literature review was to analyze the current literature to better understand the effects that KC may have on facilitating neurodevelopment of preterm infants in Neonatal Intensive Care Units (NICUs). Emphasis was placed on neurophysiologic functioning, autonomic functioning, and neurobehavioral functioning. A database search of CINAHL Plus with Full Text, PsycINFO, MEDLINE, Cochrane Central Register of Controlled Trials, and Health Source: Nursing/Academic Edition was conducted, and a total of six articles were reviewed based on their relevance and application towards this thesis. KC is a low-cost, relatively easy intervention to initiate that can have positive impacts on many aspects of preterm infant growth and maturation. There is limited research regarding the use of KC as an intervention to support neurodevelopment, especially with regards to long-term effects. Existing research supports the use of KC as an intervention to facilitate neurodevelopment in preterm infants in the NICU.

Show moreChildhood obesity is a prevalent problem in the United States. Obesity increases the risk for many diseases. Obese children are likely to become obese adults with additional comorbidities. Studies have reported mixed findings regarding associations between reading nutrition labels and improved dietary behaviors/healthy weight status. The purpose of this study is to determine whether the frequency of children reading nutrition labels is related to frequency of performing 12 dietary behaviors. De-identified baseline data from a previous quasiexperimental pilot study were analyzed. Data were collected from 4th and 5th graders (n = 42) at an after-school program. An adapted paper survey was administered to the children to measure the number of days (0�7) they read nutrition labels and performed 12 dietary behaviors over the preceding week. Due to non-normal distribution of data, non-parametric Spearman rho correlations were conducted to determine relationships between frequency of reading nutrition labels and dietary behaviors. Positive correlations were found between frequency of reading nutrition labels and eating fruit for breakfast; eating vegetables at lunch/dinner; eating whole grain/multigrain bread (p less than .05); eating fruit for a snack; eating vegetables for a snack (p less than .01). Frequency of reading nutrition labels was inversely related to drinking soda/sugar-sweetened beverages (p less than .05). Significant relationships were found between frequency of reading nutrition labels and several dietary behaviors associated with childhood obesity prevention. Findings are promising and support the need for further intervention research to determine potential direct influences of children reading nutrition labels on dietary behaviors.

When born prematurely, a neonate may face health challenges due to overall organ immaturity and hospitalization in the Neonatal Intensive Care Unit, where developmentally important maternal sounds are replaced with routine hospital noise. A potential intervention that can provide meaningful auditory stimulation these neonates lack is implementation of maternal sound interventions. These interventions replicate the intrauterine auditory environment by playing recorded maternal speech and heart sounds in the incubator. A literature review was completed to identify effects on neurodevelopmental, nutritional, and physiological measurements this intervention may have on premature neonates. A review of the literature was conducted using the databases CINAHL Plus with Full Text, Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews, MEDLINE, and PsycINFO. Search terms utilized were: Voice/Sound; preterm/prematur*; neonat*/infant*/newborn; and matern*/mother*. Results were refined using limiters of peer-reviewed, publication date of 2012-2017, and English language. Twelve articles met the criteria for review. The maternal sounds intervention was found to correlate with improved neurodevelopment in the first months of life, especially relating to auditory and language areas of the brain. Nutritional outcomes were positive, but studies were inconsistent with findings. The physiological measurements were positively affected, with strong evidence of a calming effect, and lowering of the heart rate. Results indicated recorded maternal sound interventions were associated with positive health outcomes in premature neonates. Further research with larger sample sizes and uniform study designs are needed to validate the findings.

Currently there is a glaring lack of published research regarding how goal setting is understood and experienced by mothers. The purpose of this qualitative study was to examine the self-described understanding and experiences of mothers, regarding goal setting for their preschool children.Narrative Inquiry was used to explore mothers' experiences with goal setting. A purposive sample of mothers with children in a Voluntary Pre-Kindergarten program were recruited from the central Florida area. Data were obtained from demographic instruments and personal interviews using a semi-structured guide. Interviews were coded to ensure confidentiality, audio-recorded and transcribed. The narratives were analyzed for thematic emergence using content analyses techniques.Four major themes emerged: Parental Knowledge, Barriers, Process of Goal Setting, and Provider Involvement. Parental knowledge of goal setting was varied and unique; barriers of goal setting were focused on keeping children motivated and fear of failing; each participant used a different goal setting process and noted that provider involvement was limited at best. Data analysis revealed minimal effective communication between mothers and nursing providers about goal setting. As a result, mothers utilized unique goal setting processes for their children. Despite their varied understandings of goal setting and the goal setting process, these mothers were not stifled in their goal setting efforts. Educating nurses to communicate effectively with parents about goal setting with their children and addressing barriers they might face, is important. Incorporating goal setting into routine care can be an effective strategy to help patients attain health-related goals. Future research examining the perspective of goal setting from children and other caregivers and development of interventions to aid in goal attainment is needed.

Show moreThis study seeks to influence choices made by Kindergarten students in a Title I school who receive free breakfast and lunch through a health literacy intervention with the intent of decreasing daily added sugar consumption. Fruit and milk choices, either with naturally occurring sugar (NOS) or added sugar (AS), were recorded for 70 Kindergarten students among six classes in a Title I school for ten days before a four-week health literacy intervention. Three of the classes were randomly selected to learn about 'sometimes' and 'anytime' choices through the Healthy Habits for Life curriculum delivered by representatives from Nemours Children's Hospital. Following the intervention, milk and fruit choices were recorded for ten more school days to determine differences among the control and intervention groups. Pearson Chi Square test results concluded that the health literacy intervention lead to statistically significant improvements in milk choices for the intervention group, but fruit choices were inconclusive due to inconsistencies in significance. Hierarchical loglinear analyses were run to determine if there was a difference in response to intervention between male and female students, and the results indicated that the effectiveness of the intervention was not moderated by gender. The success of this intervention for milk choices will help students who receive free school breakfast and lunch to decrease their daily consumption of added sugars, and additional research needs to be done to help students make choices that will further decrease their daily added sugar consumption.

NPs can make a significant contribution to tackling the obesity crisis. The study presented here seeks to close the gap in data related to how NPs approach weight management with their primary care patients. This study focused on a comprehensive examination of the current practice patterns of NPs related to weight management, a theoretical concept analysis of weight bias among healthcare providers, along with the results of a cross-sectional survey that investigated primary care NPs' weight management practice patterns and the relationship among attitudes, perceived barriers, self-efficacy, perceived skill, and demographic characteristics. The results from this study may be applied to provider training and education for obesity and weight management that ultimately improves patients' health outcomes.

Show moreBackground: Preschool children from single-parent households with lower socioeconomic status (SES) are absent from preschool at rates higher than any other group. Some children are chronically absent, missing more than 10% of the school year. The phenomenon of preschool attendance related to behaviors, practices, and parental decision making associated with health and illness in lower SES households has not been previously studied using grounded theory methodology.Aim: The purpose of this study was to explore decision making related to supporting attendance in a preschool of 67 children (aged 3 to 4 years) with primarily low-income, single parents and preschool teachers in South Florida. The decision making process parents and teachers face every day and the environmental supports of preschool attendance facilitated identification of factors encouraging or impeding attendance.Results and Recommendations: Focus groups and interviews with teachers, parents and administrators were conducted, and direct observation of the school attendance process and health/attendance policies were examined. Data analysis was concurrent with data collection to allow for theoretical sampling. The data analysis revealed an underlying process of (")communicating about health: benefitting children's attendance in a preschool environment.(") Supporting this theory were three themes of (a) empowerment: actions to support health, (b) trusting judgment regarding health, and (c) commitment of organization and parents to health and attendance. Recommendations for implementation of practice, policy changes, and opportunities for future research found in this unique setting were discussed to improve attendance.

Healthy People target goals for breastfeeding initiation, duration, and exclusivity remain unmet. The US Surgeon General's Office reports that lack of knowledge and widespread misinformation about breastfeeding are barriers to meeting Healthy People goals. Breastfeeding mothers are vulnerable to messages that cast doubt on their ability to breastfeed. Very little research has examined specific approaches to help people resist negative messages about health beliefs and behaviors. The objective of this quasi-experimental study was to test an intervention designed to help mothers defend their breastfeeding decisions and resist influences that attempted to persuade them to give formula to their babies. Women attending prenatal breastfeeding classes were recruited and assigned to comparison and intervention groups. The intervention was a board game based on McGuire's inoculation theory of resistance to influence. Controlling for intention to breastfed, intervention and comparison groups were examined for differences in maternal self-efficacy to resist persuasion to give formula and breastfeeding rates for initiation, duration, and exclusivity. Data analyses consisted of analysis of covariance and logistic regression. There was no significant difference between comparison and intervention groups, both groups had high self-efficacy to resist giving formula to their babies; nor were there significant differences regarding breastfeeding initiation, duration and exclusivity. The lack of significant differences may have been influenced by ceiling effects in all of the breastfeeding variables, possibly due to the high socioeconomic level of the sample. The intervention may have worked better in women who were more prone to dissuasive influence, such as those with lower education.

Show moreApproximately one-third of children in the United States are overweight or obese with increased risks for numerous physical and psychosocial comorbidities. Schools are ideal environments to address this serious health crisis and school nurses are uniquely positioned qualified healthcare providers to actively participate in childhood obesity prevention (COP). A review of the literature provided findings to identify a gap in the knowledge regarding the association between school nurse COP perceptions and practices. A modified theoretical framework based on Bandura's health promotion by social cognitive theory guided this study. The purpose of this study was to identify the key perceptions (self-efficacy, perceived benefits and perceived barriers) influencing school nurse participation in COP practices and determine associations between school nurse characteristics and COP perceptions and practices. Preliminary research was conducted to determine content validity for modified perception scales, clarity of instructions and questions, data collection and retrieval procedures, and refinement of recruitment strategies. Adequate reliability and validity was determined for modified scales measuring school nurse self-efficacy, perceived benefits, perceived barriers, and COP practices targeting individual children (child-level) and the entire school population (school-level). Florida RN school nurses (n = 171) completed self-administered anonymous questionnaires from an emailed weblink or a paper version offered at two Florida Association of School Nurses conferences. School nurses with characteristics reflecting more education engaged in more COP practices (p (<) .05) than those without education-related characteristics. School nurses with (>) 8 hours of COP education reported higher COP self-efficacy than those with none (p (<) .01). Linear regressions showed that a model comprised of self-efficacy, perceived benefits, and perceived barriers significantly explained 12.0% of the variance in child-level practices (p (<) .001) and 9.1% of school-level practices (p (<) .001). Self-efficacy explained the most variance of school nurse child-level and school-level practices (p (<) .001), and perceived barriers were inversely associated with child-level practices (p (<) .05). Four series of regressions showed that only perceived barriers partially mediated the influence of self-efficacy on child-level practices. Data analyses indicated self-efficacy and perceived barriers were key determinants of school nurse COP practices. Therefore, policy changes and educational interventions to increase self-efficacy and reduce perceived barriers may be effective in mobilizing school nurses to actively engage in COP practices.
