Show moreABSTRACT The purpose of this research is to determine the degree to which a positive experience with nursing leadership increases nurse's job satisfaction. The different values and norms of the generational cohorts result in each cohort perceiving leadership characteristics differently. Factors such as length of exposure to leadership, location, shift worked, clinical versus non-clinical positions and the presence or absence of Servant-Leadership, all have the potential to impact nursing satisfaction. Nursing satisfaction, or dissatisfaction impacts retention, further modifying nursing leadership practices. Conflict, Cohort, Servant-Leadership, and Self-Discrepancy theories were utilized to identify the relationships of generations to each other and to the leadership characteristics existing in their organizations. Two Central Florida healthcare organizations were utilized to obtain data regarding leadership characteristics, generational cohort and nursing satisfaction indicators. A total of 440 survey questionnaires were distributed, 182 were returned, a response rate of 41%. Factor Analysis utilizing principal component analysis was performed to reduce the 57 variables contained within questionnaires to one construct that represented a leadership characteristics variable. This variable was utilized to test 3 of the hypotheses. Principal component analysis was utilized to reduce 10 characteristics of Servant-Leadership, to a construct that represented a Servant-Leadership variable. Qualitative data was collected from 25 interviewees and was used to enrich and supplement the quantitative data from the survey questionnaires. Nursing leadership characteristics affect nursing satisfaction as demonstrated by this research. The more positive the perception or experience of nurses in relation to nursing leadership, the more job satisfaction increases. Even though literature states that Generation X employees exhibit less job satisfaction, due to generational specific values and norms, generational cohort did not demonstrate significance in this study. A positive perception of nursing leadership characteristics demonstrated a positive impact on nursing retention within an organization. Nurses who are satisfied with leadership characteristics tend to stay with that organization. The presence of Servant-Leadership characteristics also demonstrated a positive impact on nursing job satisfaction and retention. Leaders that demonstrate Servant-Leadership characteristics engender increased job satisfaction for their employees and increased retention of nurses for their organizations.

Show moreThe purpose of this dissertation is to examine nursing homes' compliance with state minimum nurse staffing standards and its relation to quality-of-care deficiencies. Specifically, this study, reviewing staffing standards from 50 states and the District of Columbia for the year 2007, proposes a unique algorithm to calculate the states' expected nurse staffing levels for individual nursing homes in order to investigate their compliance with the state nurse staffing standards. By using hierarchical linear modeling method, this study attempts to capture the impact of the staffing standards on actual nurse staffing levels under resource dependence perspectives. Path analysis using structural equation modeling was conducted to investigate both direct and indirect effects of the staffing standards on nurse staffing levels and quality-of-care deficiencies. The major findings were as follows: (1) nursing homes in states with higher state staffing standards for the categories of RN, LN, and total nurse were found to have higher RN, LN, and total staffing levels, respectively; (2) higher nurse staffing levels resulting from higher state staffing standards were significantly associated with better quality of care (less quality-of-care deficiencies cited) in nursing homes; and (3) state staffing standards were found to have much stronger contribution to nurse staffing levels than any other organizational or contextual factors while nurse staffing levels, particularly licensed staff, were found to have stronger contribution to quality-of-care deficiencies than any other organizational factors. The study findings suggest that if the goal is to increase nurse staffing levels for better quality, increasing the stringency of both federal and state nurse staffing standards would be the most effective way. However, the staffing standards first need technical changes to reduce their ambiguity and ensure their fairness. If the goal is to achieve better quality, merely increasing nurse staffing levels may not be effective since the variation of the quality-of-care deficiencies explained by exogenous variables was smaller than random variation 5%. If state Medicaid reimbursements can be utilized for financial incentives for better performing nursing homes, nursing homes may improve their productivity by efficiently managing organizational personnel or increasing job satisfaction among nursing practitioners. Lastly, longitudinal analysis, considering variation in length of state staffing policy implementations, is encouraged to investigate the long-term effects of state staffing standards on nurse staffing levels and quality of care.

Show moreThis study explored the correlation between the development of moral judgment and the variables of gender, age, primacy of religion, level of educational attainment, and year in the nursing program student nurses enrolled in an associate degree in nursing program who were concurrently enrolled in a bachelor degree in nursing program in the southern United States. A correlation was established with the dependent variable, or N2 index score, and the independent variable, primacy of religion. A negative correlation between the N2 index was established in students who believed that religion exerted a great influence on their lives while a positive correlation was established between the N2 index score and students who believed that religion exerted no influence on their lives. One conclusion reached was that the freshman class had a higher number of respondents who did not feel that religion was important in their lives with higher N2 index scores and a larger number of students in the Postconventional Schema of Moral Judgment. Regarding recommendations, it was suggested that ethical instruction needs to move beyond merely teaching students' ethical theories and shift towards instruction in situationally-dependent, context-driven, real-world management of ethical predicaments in practice.

Show moreEvery year, millions of medical errors are committed, costing not only patient health and satisfaction, but thousands of lives and billions of dollars. Errors occur in many areas of the healthcare environment, including the profession of nursing. Nurses provide and delegate patient care and consequently, standard nursing responsibilities such as medication administration, charting, patient education, and basic life support protocol may be incorrect, inadequate, or omitted. Although there is much literature about errors among the general nurse population and there is indication that new nurses commit more errors than experienced nurses, not much literature asks the following question: What are the factors contributing to the commission of errors, including the omission of standard nursing care, among new nurses? Ten studies (quantitative, qualitative, and mixed-mode) were examined to identify these factors. From the 10 studies, the researcher identified the three themes of lack of experience, stressful working conditions, and interpersonal and intrapersonal factors. New nurses may not have had enough clinical time, may develop poor habits, may not turn to more experienced nurses and other professionals, may be fatigued from working too many hours with not enough staffing, may not be able to concentrate at work, and may not give or receive adequate communication. Based on these findings and discussion, suggested implications for nursing practice include extended clinical experience, skills practice, adherence to the nursing process, adherence to medications standards such as the five rights and independent double verification, shorter working hours, adequate staffing, no-interruption and no-phone zones, creating a culture of support, electronically entered orders, translation phones, read-backs, and standardized handoff reports.

The purpose of this descriptive, comparative study was to explore program length, size, and admission criteria of master's of science in nursing (MSN) degree programs. Characteristics of both traditional and non-traditional programs that offered the education specialty were examined. Non-traditional programs included registered nurse to master's of science in nursing (RN-MSN) degree programs. Characteristics of students enrolled in the education track of these programs were also compared, based on participant's entry level into nursing practice. The three comparison groups included diploma prepared nurses, associate degree prepared nurses, and baccalaureate prepared nurses. Comparative and descriptive statistics with cross tabulations and frequencies, were used to determine comparisons, based on responses to questionnaire items that focused on reasons for entering the program, financial assistance, program satisfaction, goals, future plans, years of experience, nursing specialty, employment status, and demographics. The literature review found very few current studies on non-traditional and traditional MSN degree programs. Results of this study found only nine programs in the state that offered an MSN with a nursing education specialty with participation elicited from five of those nine programs. Findings revealed that within those programs were a variety of both traditional and non-traditional options that were unique in how they were organized and administered. Descriptive, comparative analysis revealed no significant differences in student characteristics of the three identified groups for those enrolled in master's level coursework in the nursing education specialty in spring of 2006. Overall, participants in the MSN programs indicated that they chose the program because they needed it for advancement, they wanted additional knowledge and skills, and the reputation and location were important. Their education was financed with personal earnings or loans. Upon completion of the program they planned to stay in their current positions and continue for a doctoral degree or specialty certification in nursing. Within 10 years they planned to be working as a faculty member in a nursing program. They were Caucasian women, aged 41-55, who lived with a significant other and children, worked more than 33 hours per week in critical care, pediatrics or education, and had more than 20 years of work experience. Findings indicated that the diversity of the MSN programs made no difference in the types of students that were attracted to the programs. These findings may be used to target recruitment efforts toward those interested in becoming nursing faculty in an effort to help alleviate the nursing faculty shortage in Florida.

The purpose of this study is to explore the presence of these types of negative attitudes as they present themselves in the nursing workforce. 520 registered nurses were contacted via email to partake in a survey assessing homophobic attitudes and perceptions regarding nursing care of LGBT persons. A total of 27 registered nurses responded and the resulting data were analyzed using descriptive statistics. A majority of registered nurses were female (89.3%), greater than 40 years of age (75%), white (75%), heterosexual (96.4%), and Christian (67.9%) with a Bachelor's degree or less (57.1%). Homophobia scores averaged 27 on a scale from 12-60, higher scores translating to greater homophobia levels. This value is on the lower end of the scale, which interprets to lower levels of homophobia among the participants. While some of these scores did show the existence of negative attitudes toward LGBT individuals among participants, further investigation is needed with a larger, more representative sample. As a result, it is difficult to determine whether LGBT relations are improving with registered nurses.

Because the etiology of fibromyalgia is unknown, it is difficult to treat. Research shows that medication alone is insufficient to treat the symptoms of fibromyalgia and that complementary therapies are required to fully manage this disorder. The purpose of this research was to determine what complementary therapies patients with fibromyalgia were currently using. The sample for the survey was taken from fibromyalgia support groups throughout the state of Florida. A total of 15 people diagnosed with fibromyalgia participated in the survey. The most commonly used complementary therapies included diet and relaxation techniques. The therapies that were rarely or never used were hypnotherapy, Tai Chi, and Chi (Qi) Gong. These findings provide evidence to support client education concerning the use of complementary therapies for individuals with fibromyalgia. Research with larger samples is recommended to provide further evidence of the effectiveness of complementary therapies for this patient population.

Show moreThe response of community members and public institutions to the crimes of rape and sexual assault have been, historically, disappointing. Victims have felt forced into silence, ashamed to speak out about their experiences. More recently, with the rise of social media use and the #me-too movement, attention and interest has been garnered for this unique class of crimes and victims. It has been discovered that victims of sexual assault who report their assault to community members, police investigators, and health care workers are at risk of secondary victimization. This is a series of attitudes, behaviors, and practices that retraumatizes, shames, and blames victims of sexual assault. These behaviors contribute to increased levels of Post-Traumatic Stress Disorder (PTSD), depression, anxiety, shame, self-blame, lack of perpetrator prosecution, and more. The purpose of this thesis was to evaluate the causes and effects of secondary victimization, its extent and prevalence, specifically how young adult females were affected by this issue, and any potential programs or solutions that may reduce or limit secondary victimization. This aim was achieved by reviewing and synthesizing thirty-two research articles related to issues of women's health, secondary victimization, and sexual assault or rape. Several programs were found to show promise for reducing this phenomenon, including sexual assault nurse examiner programs, victim advocates, professional counseling, and improving the education of police investigators and others involved in interactions with sexual assault victims.

Show moreThe lesbian, gay, bisexual, and transgender (LGBT) community is a unique population that has specific health issues and health care needs associated with lifestyle behaviors that increase risk for certain diseases. Health concerns include mental and behavioral health, issues associated with gender identity and relationships (i.e. intimate partner violence), sexually transmitted infections, and chronic illnesses. The research suggests poorer health outcomes for the LGBT population compared to heterosexual and/or cisgender counterparts. Most research conducted with LGBT populations occurred in more populated urban settings with very few studies focusing on this population in the rural context. Consequently, there is a paucity of information on the health care concerns of the rural LGBT population. Considering the information gap, this integrative review of 14 research articles focused on health-related issues of the LGBT population in rural regions. The findings revealed rural LGBT persons experience disparities in accessing health care and support services; coupled with health care providers who often were not culturally competent; and, sometimes, unfamiliar with evidence-based health care protocols when caring for the LGBT patient. Implications for nursing research, education, practice, and policy and study limitations are highlighted.

The intent of this thesis was to explore nurse perceived barriers to effective nurse-client communication. A literature review was conducted and nine articles were identified as addressing nurse perceived barriers to communication. Four major barriers were identified: nurse comfort and knowledge, environment, time, and culture and language. Research on interventions to address nurses' perceptions of barriers to effective nurse-client communication may provide a better understanding of communication barriers and address issues created by ineffective communication with clients.

Background: Human papillomavirus (HPV) infections are the most common sexually transmitted infections in the world today. HPV has been identified as the cause of both cancer and genital warts. A vaccine to prevent select types of HPV infection was developed and approved for males and females ages 9 to 26. Show moreBackground: Human papillomavirus (HPV) infections are the most common sexually transmitted infections in the world today. HPV has been identified as the cause of both cancer and genital warts. A vaccine to prevent select types of HPV infection was developed and approved for males and females ages 9 to 26. This vaccine has not been universally accepted by healthcare providers, parents, or the public and has lower vaccination coverage rates than other recommended vaccines for the same age group. Multiple studies addressing the knowledge, perceptions, and beliefs of providers and parents regarding the vaccine have taken place. Only a few studies with the nursing profession are found in the literature and they do not look at the vaccine in regards to males. Nurses are seen as one of the most trustworthy of professions. What nurses know, perceive and believe can impact both patients and those they come in contact with on an informal basis. Objective: The purpose of this study is to determine knowledge, beliefs, and perceptions of registered nurses about the human papillomavirus and associated vaccination for males. Methodology: This study took place in February of 2013 and used a descriptive correlational design with a cross-sectional survey. The survey was administered online to alumni of the University of Central Florida (UCF) College of Nursing, members of the Theta Epsilon chapter of Sigma Theta Tau International Honor Society, and current graduate nursing students and faculty of UCF College of Nursing. There were one hundred and twelve Registered Nurses that participated in the study. Findings: Knowledge of HPV varied among respondents. The vast majority knew that HPV was a common sexually transmitted infection however less were knowledgeable on specific iii information about the vaccine. Less than eighty percent of study participants knew that the vaccine was available for both males and females. Close to ninety percent of respondents agreed that vaccinating males would be important to protect their future partners from complications associated with HPV, such as genital warts and cervical cancer, and to protect themselves from future complications, such as genital warts and cancers. Participants with doctoral degrees possessed a significantly more favorable attitude (p<0.001) towards vaccine use in males than those with a bachelors degree. Conclusions: Nurses in this study were knowledgeable about specific HPV information but were less knowledgeable about the extent of infection seen in males or the availability of the vaccine for males. This study found inaccuracies and lack of knowledge among Registered Nurses that demonstrates the need for education about HPV and the associated vaccine. While the attitudes of the respondents reflect favorably toward the vaccine, there is still a need for education among this population.

This project explores nursing attitudes toward single-use medical devices, evaluates current recycling practices and examines whether student nurses would be likely to use reprocessed SUDs in their practice if given the option. METHODOLOGY: After obtaining IRB approval, students enrolled in nursing research courses were invited to participate in this exploratory study. 157 undergraduate nursing students completed the 46-question survey. Descriptive statistics were used to analyze the survey results, independent t-tests were used to compare groups and content analysis was used to analyze open-ended responses. RESULTS: The typical student was female, age 30, licensed as a registered nurse and enrolled in the RN to BSN program. Most students (84.7%) viewed themselves as environmentally conscious, and most recycle at home (75.5%) and at work (54.7%). Most agreed (96.8%) that hospitals produce a lot of hazardous waste and that it is the hospitals responsibility for environmentally friendly waste disposal. The majority also agreed (80%) that nurses have the ability to impact waste production at a hospital. More than half (67.7%) agreed that SUD disposal contributes to environmental pollution and many (76.6%) felt that nurses should be responsible for environmental health concepts. Most (81.6%) felt that SUDs should be thrown out after one use and few (28.5%) felt that SUDs can be reused if sterilized. Most (74.0%) also believed that SUD reuse contributes to hospital acquired infections, but a little more than half (56.3%) were willing to reuse a SUD that had only touched intact skin if sterilized for reuse. Additionally, most respondents (79.1%) would consider joining a green team at work. Those who recycle at home were more likely to identify as environmentally conscious than those who do not recycle at home. No generational differences existed when considering environmental consciousness. Generation X was more likely to recycle at home than Generation Y, but no generational differences existed when analyzing work recycling habits. Generation X was also more likely to see single-use device disposal as contributing to environmental pollution than Generation Y. Home recyclers were more likely to agree that nurses have the ability to decrease the amount of hospital trash production, and more likely to join a green team than non-recyclers. They also believed that SUD disposal contributes to environmental pollution, SUDs can be reused if sterilized, and disagreed that SUD reuse contributes to hospital acquired infections when compared to those who do not recycle at home. Discussion: While most students agree that hospitals produce large amounts of waste and should be responsible for the disposal of it in an environmentally friendly manner, most are hesitant to use reprocessed SUDs as a means to make the hospital more environmentally friendly. Student responses indicated the largest perceived barriers to SUD reuse were fears of inadequate sterilization and fears of the spread of disease. CONCLUSIONS: Most students, especially home recyclers, believe themselves to be environmentally conscious and most were willing to consider reusing some SUDs. Translating this belief into action can happen through education in line with the Scope and Standards of practice for nursing, as well as establishing the safety of SUDs through further research.

A significant relationship was identified between the composite variables representing engagement and incivility. Specifically, the composite engagement variables representing active and collaborative learning, student-faculty interaction, student effort, and academic challenge were positively related to the composite incivility variable reflecting the consideration of disruptive student behavior. Data analysis determined that the most disruptive classroom behavior reported were students holding distracting conversations. The use of computers for non-classroom activities was cited as the most frequently observed disruptive act. The study examined the presence of any differences in the levels of student engagement or incivility between first- and second-year students. No differences in either of these two constructs were identified. The study results suggest a relationship between incivility and engagement and denote the most prevalent and disruptive nursing student behaviors.

When together and in close proximity, a women and her infant have access to one another to allow for mutual caregiving or caregiving on cue. A new measure entitled the Mother-Infant Togetherness Scale (MITS) was developed based on a review of the literature and conceptual framework of Mother-Newborn Mutual Caregiving. The MITS is a 35-item instrument composed of four subscales that measures the timing, duration, and intensity of togetherness of the mother-infant dyad during entire hospitalization. The purpose of this multiphase study was to obtain support for the validity of the MITS. Phase 1 determined the content validity at the scale (S-CVI), subscale, and item level (I-CVI) with a panel of expert judges. The final sample for the content validation consisted of 7 judges from medicine (n = 2), maternal-child nursing (n = 1), nursing research (n = 3), and social work (n = 1). Judges were instructed to use a 4-point Likert scale to rate the relevance of each item (I-CVI) to the construct of togetherness. The S-CVI was calculated from the mean I-CVI scores. The CVI-S of .88 was just slightly below the desired CVI-S ((>) .90). Of the four subscales, all had adequate CVI ((>) .90) at the subscale level except the delivery affective subscale (CVI = .74) and postpartum affective subscale (CVI = .89). The delivery events and postpartum events subscales had satisfactory CVI scores (CVI (>) .90), 1.00 and .94, respectively. The CVI-I results identified a total of seven items on the affective subscales that did not meet the desired I-CVI ((>) .78). Phase 2 pre-tested the readability and understandability of the MITS among eight postpartum women. During the interviews, the women were asked to complete the MITS and provide opinions about the readability and understandability of the directions and items. The audiotapes were transcribed word for word, reviewed for thematic content, and revisions made to the study instrument accordingly. This same sample of postpartum women participated in the content validation of the delivery affective subscale (items #4a-j) and postpartum affective subscale (items #17a-j). The I-CVI results identified that a total of six items on the affective subscales had a CVI-I of .75, just slightly below the desired I-CVI ((>) .78). Scale items were deleted or revised and the instrument retested until the desirable CVI at the scale and subscale level was achieved. Phase 3 used a descriptive study design to examine women's ability to accurately self-report birth events on the MITS delivery events subscale at 4 weeks postpartum, as compared to observer-collected data obtained at delivery to determine the most valid mode of administration. A purposive sample consisted of 45 women having delivered at a community hospital in southwest Florida. The research team completed the MITS delivery events subscale immediately after delivery. Women were sent the MITS for completion 4 weeks after delivery. McNemar Chi-Squares were (?) were calculated from the self-reported MITS delivery events subscale scores and the observer-collected MITS delivery events subscale scores. No significant difference (p (<) .05) was found supporting self-reported mode of administration for the MITS. Phase 4 is in-progress and evaluates the reliability and validity of the MITS subscale and total scale scores. The interim analysis was performed on a sample of 113 postpartum participants (composed of the final sample of 31 participants from Phase 3 and the first 82 participants from Phase 4) having delivered at three of the four participating hospital study sites. Adequate internal consistency reliability was found at the scale level with Cronbach's alpha (? = .89) and split-half reliability results (? = .79 (-) 81, r = .83 - .88). Of the 35 MITS items, 10 items (28.6%) were found to have item-total correlations less than .30, arguing against treating MITS items as a single total scale measure. Good internal consistency was found at the delivery events subscale level (? = .78). Exploratory factor analysis (EFA) identified a two-factor solution. The two factors were named Taking In and Taking Control and had internal consistency reliability.79 and .65, respectively. Additional work needs to be done to improve the internal consistency of the Taking Control factor. The postpartum events subscale also had low internal consistency (? = .58). This subscale was not factor analyzed because the item response data did not meet the criteria for factor analysis. The items on the postpartum events subscale were assessed to be unique, singular, heterogeneous items that did not correlate well with other items. These results are conceptually logical given the nature of what the items are measuring (occurrence/intensity of specific events in time). The delivery affective subscale had good internal consistency reliability (? = .85) and a two factor solution. The two factors, named Feelings At Delivery and Delivery Concerns, had adequate internal constancy (? = .81 and ? = .80, respectively). The postpartum affective subscale had good internal consistency reliability (? = .92) and a one factor solution. Results for known groups testing based on feeding type and mode of delivery found all group differences were in the predicted direction. Higher scores were found for mother-infant dyads who breastfed than for mother-infant dyads who bottle fed. However, only group differences for the events subscales were substantive and statistically significant (p (<) 001.). Higher scores were found for mother-infant dyads who experiencing a vaginal delivery than for mother-infant dyads who experienced a cesarean delivery. Group differences were substantive and statistically significant (p (<) .01) for three of the four subscale scores. A post hoc power analysis on the means and standard deviations from the interim analysis and the between-groups comparison effect size observed for feeding type (d = .50) found a sample of 45 adequate to have statistical power at the recommended beta of .80 and alpha of .05. The post hoc power analysis on the effect size for mode of delivery (d = .75), found a sample of 156 are needed to obtain statistical power at the recommended beta of .80 and alpha of .05. Therefore, the desired sample size of 200 women for the final analysis is adequate to obtain statistical power. A third known group testing for the variable of central nursery availability could not be performed with the interim analysis data because no participants in the interim analysis sample reported this experience. However, this analysis will be performed with the final data set. This is the first study to operationalize togetherness during the entire hospitalization and to include all dimensions of the construct. The findings from this multi-phase study provide initial support for the reliability and validity of the MITS. Although the results from Phase 4 are interim and therefore tentative, they provide preliminary psychometric evidence for construct validity.

Schools are ideal environments to address this serious health crisis and school nurses are uniquely positioned qualified healthcare providers to actively participate in childhood obesity prevention (COP). A review of the literature provided findings to identify a gap in the knowledge regarding the association between school nurse COP perceptions and practices. A modified theoretical framework based on Bandura's health promotion by social cognitive theory guided this study. The purpose of this study was to identify the key perceptions (self-efficacy, perceived benefits and perceived barriers) influencing school nurse participation in COP practices and determine associations between school nurse characteristics and COP perceptions and practices. Preliminary research was conducted to determine content validity for modified perception scales, clarity of instructions and questions, data collection and retrieval procedures, and refinement of recruitment strategies. Adequate reliability and validity was determined for modified scales measuring school nurse self-efficacy, perceived benefits, perceived barriers, and COP practices targeting individual children (child-level) and the entire school population (school-level). Florida RN school nurses (n = 171) completed self-administered anonymous questionnaires from an emailed weblink or a paper version offered at two Florida Association of School Nurses conferences. School nurses with characteristics reflecting more education engaged in more COP practices (p (<) .05) than those without education-related characteristics. School nurses with (>) 8 hours of COP education reported higher COP self-efficacy than those with none (p (<) .01). Linear regressions showed that a model comprised of self-efficacy, perceived benefits, and perceived barriers significantly explained 12.0% of the variance in child-level practices (p (<) .001) and 9.1% of school-level practices (p (<) .001). Self-efficacy explained the most variance of school nurse child-level and school-level practices (p (<) .001), and perceived barriers were inversely associated with child-level practices (p (<) .05). Four series of regressions showed that only perceived barriers partially mediated the influence of self-efficacy on child-level practices. Data analyses indicated self-efficacy and perceived barriers were key determinants of school nurse COP practices. Therefore, policy changes and educational interventions to increase self-efficacy and reduce perceived barriers may be effective in mobilizing school nurses to actively engage in COP practices.

The phenomenon of interest for this study using focused ethnographic method is the self-described experience of coping and adaptation associated with workplace stress of registered nurses working 12-hour shifts employed in acute care hospital facilities in east central and central Florida. Three aspects of the phenomena were examined: the self-described experiences of stress, the manner in which coping skills are acquired, and the manner in which adaptation strategies are developed by experienced bedside nursing working 12-hour shifts in acute care hospital facilities. The purposive sample included nine female bedside nurses with five or more years' experience, working 12 hour shifts in acute care hospital facilities on bedside units, with patient ratios of 4:1 or greater. Data were collected using semi-structured, digitally recorded interviews at mutually convenient locations. The qualitative data were analyzed using inductive, constant, comparative process of coding, sorting, generalizing, and memoing to guide exploration and identify emergent themes and patterns. The predominant theme of stress emerged as the overwhelming sense of duty to the patient. Additional themes of coping and adaptation were noted. Recommendations for research, education, practice and policy are offered to support a healthy and sustainable nursing workforce.

The potential for confounding variables was discussed. There were no significant differences found in the relationship between orientation participation and first-year retention rates or orientation participation and persistence to graduation. Data analysis also determined that student age did not influence the prediction of first-year retention rates for students who did or did not participate in the orientation program. The results of this study suggest that there is no relationship between a program-specific orientation program for associate degree nursing students and their first-year retention and persistence rates.
