Show moreHealthcare quality remains a significant issue due to fragmentation of care in our complex U.S. healthcare systems. While coordination of care is foundational to healthcare quality as well as identified as a National Priority, fragmentation and uncoordinated care continues to afflict our systems. The purpose of this study was to explore the relationship between relational coordination and adverse nurse sensitive patient outcomes, namely hospital acquired pressure ulcers, patient falls with injury, catheter- associated urinary tract infection, and central line-associated blood stream infection. A retrospective correlational survey design using cross sectional data was used to conduct this quantitative study. An electronic relational coordination survey was sent to 1124 eligible registered nurses from 43 nursing units within a 5-hospital magnet-designated healthcare system to gather their perception of the strength of relationship and communication ties of their work team. The nurse practice environment as well as nurse education were control variables. With 406 nurses who completed the survey (36% response rate), findings revealed that the stronger relational coordination ties are amongst the healthcare team, the lower the rate of adverse nurse sensitive patient outcomes as indicated by their inverse relationship. (rs=-.31, p=.050). In a Negative Binomial Regression model, relational coordination was a significant predictor (?-1.890, p=.034) of nurse sensitive patient outcomes whereas nurse education level (p=.859) and nurse practice environment (p=.230) were not. Data affirms that relational coordination, a relationship and communication intensive form of coordination does impact patient outcomes. This research provides significant information to health care leaders and institutions with goals of improving patient care outcomes through enhancement of coordination of care and optimization of healthcare teams.

Additionally, previous studies demonstrated the importance of mothers' adverse childhood experiences in the relationships among some of these constructs. To expand on the existing knowledge, the present study examined the relationships among mothers' temperament, reflective functioning, attributions, and specific parenting behaviors, with reflective functioning and attributions serving as two potential mediating variables in these relationships. A community sample of 224 diverse mothers of young children who were between 2- to 5-years of age rated their own adverse childhood experiences, temperament, reflective functioning, attributions, specific parenting behaviors, satisfaction with their maternal role, and psychological symptoms. Additionally, mothers rated their children's temperament, behavior problems, and adaptive functioning. Statistical analyses were conducted on the overall sample as well as on a subsample of participants who reported a high exposure to adverse childhood experiences. Correlational analyses indicated a variety of significant relationships among the variables of interest. Next, mediational analyses indicated that mothers' attributions mediated the relationship between mothers' temperament and parenting behaviors in both the overall sample and the subsample of mothers who reported high exposure to adverse childhood experiences. Further, hierarchical regression analyses demonstrated that a number of maternal characteristics predicted young children's outcomes. Overall, this study identified unique predictors of mothers' parenting behaviors and of mothers' perceptions of the outcomes experienced by their young children. Most importantly, this study highlighted the importance of serving families as a whole when wanting to provide lasting improvements to individual and family functioning through intervention services.

Furthermore, racial and ethnic minorities underutilize mental health services and have low retention when engaged in services, highlighting the need for counseling professionals to empirically explore factors that may be contributing to client engagement of the counseling process. Exploration of multicultural competence and working alliances may increase understanding of the therapeutic factors that influence client outcomes. The purpose of this research study was to investigate relationships between multicultural competence, working alliance, and client outcomes as perceived by counselors-in-training and their clients (N = 191; n = 72 counselors'-in-training, n = 119 clients). The Tripartite Model of Multicultural Counseling (Arredondo et al., 1996) was used as the primary theoretical framework in which the study is grounded. This investigation explored clients' perceptions of their counselors'-in-training ' multicultural competence as measured by the Cross-Cultural Counseling Inventory ([CCCI-R]; LaFromboise, Coleman, (&) Hernandez, 1991), the working alliance as measured by the Working Alliance Inventory- Short Revision ([WAI-S]; Horvath (&) Greenberg, 1989; Tracey (&) Kovocivic, 1989) and prediction on client outcome as measured by the Outcome Questionnaire 45.2 ([OQ 45.2]; Lambert (&) Burlingame, 1996), after controlling for social desirability (as measured by the (Social Desirability Scale-Short Form [SDS; Reynolds,1982]). This investigation also examined if there were any differences in clients' and counselors'-in-training perceptions on multicultural competence (as measured by the CCCI-R) or the working alliance (as measured by the WAI-S). Results from the investigation indicated that counselors'-in-training perceptions of their multicultural competence was a predictor of client outcomes. However, counselors'-in-training perceptions of the working alliance or clients' perceptions of their counselors'-in-training multicultural competence and the working alliance were not predictors of client outcomes. Positive relationships between clients' and counselors'-in-training perceptions of counselors'-in-training multicultural competence and the working alliance were found. The results of this investigation contribute to a gap in the counseling literature on multicultural competence, the working alliance, and client outcomes. A review of the literature on the constructs of interest, research methodology, data analysis, results and implications are discussed.

Show moreGiven the substantial monetary and nonmonetary costs that both employees and organizationscan incur as a result of perceived workplace discrimination, it is important to understand the outcomes of perceived workplace discrimination as well as what moderates the discrimination-outcome relationship. While other meta-analyses of perceived discrimination have been published, the current meta-analysis expands prior meta-analytic databases by 81%, increasingthe stability of the estimated effects. In addition, several prior meta-analyses have not focused exclusively on workplace discrimination. Consequently, the purpose of this meta-analysis is to provide a comprehensive quantitative review of perceived workplace discrimination, its consequences, and potential moderators of these relationships. Results showed that perceived workplace discrimination was related to decreased job satisfaction, reduced organizational commitment, greater withdrawal, and more perceived organizational injustice. Further, perceived workplace discrimination was associated with decreased mental health and physical health, lower ratings of life satisfaction, and increased work stress. Moderator analyses provided some evidence that perceiving the general presence of discrimination in one's organization may bemore detrimental than perceiving oneself to be personally targeted by discrimination at work.Additionally, moderator analyses provided some support that interpersonal discrimination maybe more detrimental than formal discrimination for some outcomes and that there may bedifferences in the perceived workplace discrimination-outcome relationships across different countries. The implications for workplace discrimination research and practice are discussed.

Show morePrevious research has demonstrated a relationship between the built environment and social and transportation outcomes when comparing traditional and conventional suburban neighborhoods, but much remains to be learned about whether New Urbanism can produce similar results. Among studies where new urban neighborhoods have been assessed, most have centered on regions with highly-utilized public transit systems and with climates that are amenable to utilitarian physical activity. This research sought to build on the existing research base through direct comparison of new urban and conventional suburban neighborhoods in central Florida, a region with an under-developed transit system and a climate that renders utilitarian physical activity impractical. Further, this research sought to lend greater insights into neighborhood selection factors across neighborhood types.(<)p(>)(<)p(>)A mixed-methods, single-case design was utilized to evaluate one new urban and one conventional suburban neighborhood in the central Florida region. Regional new urban neighborhoods were subjectively rated for adherence to tenets of the Charter of the New Urbanism, with the neighborhood (Celebration, in Osceola County, FL) found to most closely adhere to these tenets selected as the experimental group for the study. A socio-demographically comparable conventional suburban neighborhood (Sweetwater, in Seminole County, FL) was selected as the control group. Quantitative methods consisted of a household survey issued to 250 randomly- and convenience-sampled addresses in each neighborhood, followed by regression analysis to evaluate study hypotheses. Qualitative methods employed analysis of open-ended survey responses, detailed case studies of selected neighborhoods, and resident interviews. The household survey yielded net response rates of 15.79 percent and 25.50 percent for experimental and control neighborhoods, respectively, and a mean cross-neighborhood response rate of 20.64 percent. Twenty resident interviews (10 per neighborhood) were conducted. Quantitative and qualitative findings were compared to collectively address research questions.(<)p(>)(<)p(>)Regression results indicated no statistically significant difference between neighborhoods in attitudinal and behavioral components of community participation, in vehicle miles driven per week, or utilitarian physical activity frequency. However, results indicated that new urban residents had more positive attitudes toward utilitarian physical activity than conventional suburban residents and that attitudes toward community participation and utilitarian physical activity were positively correlated with associated behaviors. Qualitative findings provided substantial individual- and environmental-level insights to factors impacting evaluated attitudes and behaviors, and supported some quantitative findings while not aligning with others. Neighborhood selection factors were found to be quite different across neighborhoods: Celebration residents identified neighborhood social atmosphere and connection to the Walt Disney Company brand as top contributors to their selection decision, while Sweetwater residents expressed that access to quality schools was the most important factor in their selection decision. Qualitative findings indicated that car culture and climate within the central Florida region diminished both attitudinal and behavioral components of utilitarian physical activity across neighborhood types.(<)p(>)(<)p(>)This research expanded the understanding of the social and transportation outcomes of New Urbanism, particularly with respect to the central Florida region. While case and quantitative limitations may have impeded the ability of this study to draw decisive conclusions about research questions, distinctive themes regarding social and transportation outcomes were identified. Findings of this research supported those of some prior studies while contradicting others, indicating that further exploration is needed to establish a firm understanding of the capabilities of new urban development to achieve desired outcomes, and of regional characteristics that may influence these outcomes.

The study involved the collection and analysis of retrospective data to determine differences in birth weight of infants, tobacco use during pregnancy, and Apgar scores of infants. Additionally, a questionnaire, focus group discussions, and follow-up interviews were conducted with former students of the healthy pregnancy curriculum to examine the adolescent mothers' perceptions of the major concepts in the healthy pregnancy curriculum. The participants answered questions regarding how important they thought each major concept is for inclusion in the healthy pregnancy curriculum, how much they learned about each major concept, and how helpful that information was in their own experiences. Analyses of the data did not show statistical differences between adolescents who successfully completed a healthy pregnancy curriculum and those who did not complete a healthy pregnancy curriculum regarding differences in birth weight of infants, tobacco use during pregnancy, and Apgar scores of infants. These findings of non-significance may be due to the small number of participants (n=50), non-participants (n=149), and the limited duration of the study data (1999 to 2003). A larger population over a longer period of time might yield different results. The findings from the qualitative data provided by the seven former students suggest that pregnant adolescents who successfully complete the class perceive the components of the healthy pregnancy curriculum as valuable and important. Topics that were indicated as particularly important were The Birth Process, Nutrition, Decision Making, and Family Planning. Participants further indicated changes in their attitudes for all ten topics and changes in behaviors in the areas of Human Reproduction, Nutrition, Health-Care Practices, Environmental Effects on the Unborn Baby, and Decision Making.

Show moreThe purpose of this study was to examine if differences exist in student learning outcomes between students who are instructed in a co-taught or non co-taught environment in secondary social studies classrooms implementing Content Enhancement Routines (CER). This study examined student and teacher data from seventeen matched pairs of co-taught and non co-taught middle and high school general education social studies teachers who participated in professional development in CER and professional development in co-teaching if applicable. Of the 34 participating teachers, 23 were visited by school district personnel to verify implementation of CER. Five co-teaching teams, each consisting of a general and special educator, completed a Coteaching Rating Scale (CtRS) (n=10) to analyze the level of co-teaching occurring in the classroom (beginning, compromising or collaborating stage). A systematic sample of students (n = 907) completed a CER Student Perception Survey to examine perceived differences of the use of CER in co-taught and non co-taught social studies classrooms. Student state assessment scores (n = 318) in co-taught and non co-taught classrooms were analyzed to distinguish differences in student learning gains. Specifically this study investigated if differences in student performance occur when a special educator is present in the classroom. Results indicate that although there was no statistically significant difference in student learning outcomes between the two settings, the impact of teacher preparation, professional development and the implementation of CER in the secondary social studies classroom may be determining factors in student success.

Of these patients, 400,000-500,000 will die annually giving the ICU the distinction of having the highest rate of mortality and complications in the hospital setting. Studies have demonstrated that one in ten patients who die each day in ICUs might survive if intensivists were present to manage clinical care and direct treatment plans (Randolph & Pronovost, 2002; Dimick, Pronovost, Heitmiller & Lipsett, 2001; Pronovost et al., 2002). The utilization of supplemental remote telemedicine has been investigated as a means of compensating for the limited resource of intensivists (Breslow et al., 2004; Rosenfeld et al., 2000). One specific use of this technology, the electronic intensive care unit or eICU®, has previously demonstrated the potential to improve physiological and economic outcomes in ICU patients through the use of integrated decision-support and patient data systems. The present study focuses on the eICU® as a 21st century technology capable of improving the quality of patient care and identifies those factors most likely to impact the success of this clinical intervention. This research utilizes a non-experimental pre-and post-intervention study design and examines patient data collected on all admissions to five ICUs managed by two regional tertiary care hospitals during a 36-month time period. Each ICU is equipped with eICU® software systems that allow intensivist surveillance of patients from a remote facility. The data is analyzed using both structural equation modeling and decision tree regression modeling to statistically evaluate the effects of patient, hospital and unit characteristics on proximal and distal outcomes in ICU patients. As the development of clinical complications subsequently affects patient length of stay, cost of stay, and mortality, it becomes increasingly imperative to seek interventions capable of reducing the risk of unfavorable patient outcomes. This study closely examines one such intervention, the eICU®.

Show moreBackground: Historically, patients with cancer have been perceived as poor candidates for ICU admission. General ICU admission criteria lists cancer patients as low priority in ICU admission depriving them of the care they rightfully deserve. The purpose of this literary synthesis was to examine ICU admission criteria, risk factors, and outcomes of ICU admission in relation to hematological and solid tumor cancers and discuss ways that practitioners and nurses can educate patients with cancer and their families on appropriateness of ICU care. Methods: A total of 768 articles were found in a literature search including all literature from 2005 to 2016 from all countries using the databases CINAHL Plus, MEDLINE, PsycINFO, and Academic Search Premier. These were further narrowed down based on relevancy by topic or reading abstracts. A total of 13 articles utilizing the inclusion and exclusion criteria of the literature search were included in the final literature synthesis. Results: In addition to general ICU admission criteria several other criteria and scores can be helpful in admitting patients with cancer to the ICU including cancer specific criteria, mortality predictor tools, performance status, and ICU trials. Mortality predictors, in combination with other patient characteristics, demonstrated effectiveness to predict outcomes in patients with cancer. Survival rates in hematological and solid tumor cancers have improved from the past, and lower prognostic scores can predict who will have better outcomes. Conclusion: Cancer specific criteria, mortality predictor tools, performance status, and ICU trials in addition to general ICU criteria should be used for admission of cancer patients into ICU. Practitioners and nurses should become familiar with the newest outcomes in patients with cancer to make collaborative informed decisions about ICU admission.

Despite the collective research done on these variables, no study, until now, has looked at retrospective bullying, humanity-esteem, attachment, behavior problems, and self-esteem all together. This study not only looked at the relationships among these variables but also the role that humanity-esteem and attachment served between victimization, later behavior problems, and later self-esteem. One hundred thirty-six participants completed five questionnaires assessing experiences of retrospective bullying, humanity-esteem, current attachment relationships, behavior problems, and self-esteem. The results of this study indicated that participants who reported having been bullied previously also endorsed internalizing and externalizing problems as well as low self-esteem. Further, humanity-esteem and attachment both served as significant predictors of victimized individuals' behavioral problems and self-esteem. Such findings suggested that a higher view of humanity and secure attachment may serve as a protective factor against the negative outcomes that may be related to having been bullied. The importance of studying the relationships among these variables is discussed further.

This training program consisted of one initial four-hour training session, two hours of follow-up supervision, and formal feedback via MITI 3.0 scores. Counselor-trainee skill was assessed via the MI Knowledge Questionnaire, the Helping Responses questionnaire, and the Motivational Interviewing Treatment Integrity code 3.0. In addition, the following instruments were used to assess client outcomes ; attendance via observation assessments, the Session Summary, the Outcome Questionnaire-45.2, and the Client Satisfaction Questionnaire. The sample consisted of 43 graduate-level counselor trainees in their first or second semesters of practicum and 81 adult clients being seen in a university-based graduate student-training counseling clinic. Participants were purposefully assigned to either a treatment or control group. The counselor trainees in the treatment group received the unique training program, follow-up supervision, and feedback whereas the participants in the control group received a four-hour orientation to the student-training counseling clinic. There were four primary hypotheses proposed for analysis within this study (a) How does a brief training in Motivational Interviewing given to counselor trainees affect their ability to accurately perform MI?, (b) How does a brief training in Motivational Interviewing given to counselor trainees affect client functioning?, (c) How does a brief training in Motivational Interviewing given to counselor trainees affect client adherence?, and (d) How does a brief training in Motivational Interviewing given to counselor trainees affect client satisfaction with treatment? The statistical analysis of these variables yielded significant findings. Specifically, counselor-trainee skill in MI significantly improved in the treatment group as compared to the control group as assessed by the MITI with regard to the following variables: evocation, collaboration, autonomy/support, empathy, direction, MI non-adherent giving information, closed questions, simple reflections, complex reflections, total reflections, global scores, ratio of open questions, ratio of reflections, and ratio of MI adherent behavior. In addition, analysis revealed significant between group differences with client attendance. Specifically, between group-differences suggested that clients in the MI treatment group attended more sessions, missed fewer sessions, and completed therapy more frequently than clients in the control group. Detailed procedures and results as well as implications for the counseling profession and future research are explored within this study.

These indicators have remained at exceptionally high rates in the United States despite the levels of resources and technology. In the quest to understand that discrepancy, among the ranges of theories and models for explaining poor pregnancy outcomes an emerging concept is coming to attention: social capital. In order to test whether maternal social capital has an impact on pregnancy outcome, women in a Healthy Start program were surveyed over a 13-month period to assess their social capital levels and then their pregnancy outcomes. What emerged was that maternal social capital can predict up to 47% of the variance in pregnancy outcome. That is a powerful research result considering that previously there has been no literature tracing a link between maternal social capital and pregnancy outcome. In this study, maternal risk factors adversely affect up to 30% of the variance in pregnancy outcomes. Previous research has focused on maternal risk factors as the primary reason for high rates of preterm delivery, low birth weight, and infant mortality in the United States. However, this research found that in the sample of women at risk for adverse pregnancy outcomes, maternal risk factors had a very strong influence on maternal social capital (R-square=65%) while their effects on pregnancy outcomes were about half of their effects on social capital. This result suggests that social capital mediates the effects of maternal risk factors on pregnancy outcomes. It appears that one of the reasons that the high rates of adverse pregnancy outcomes in the United States have remained a mystery is that maternal social capital has not been taken into account.

Federal and state policies impacting the population, campus support initiatives at colleges nationwide and the concept of resilience were also explored. In conjunction with the Florida Department of Education's Division of Accountability, Research and Measurement, the Florida Department of Children and Families provided access to a dataset compiled by the Community College and Technical Center MIS department. This file contained enrollment information for foster care youth utilizing one of three tuition exemptions to fund their education-related expenses. While all personal identifiers were eliminated prior to sharing the file, information within the document included student age, gender, race/ethnicity, academic discipline, and degree being pursued.Results of this study yielded some statistically significant differences across tuition exemption type. After examining relationships between gender and race/ethnicity and tuition exemption type, no statistically significant results were found. However, statistical significance was found after examining the relationships between academic degree being pursued and academic major/discipline and tuition exemption type. Many factors impact the experiences of foster care youth in the college classroom. These should be considered when developing programming, policy, and support services aimed at encouraging their success.

Show moreThe first semester of practicum is a difficult time for counseling students as they learn to integrate knowledge and theory into clinical practice, often evoking high levels of anxiety (Barbee, Scherer, (&) Combs, 2003; Ronnestad (&) Skovholt, 1993) and limiting counselor self-efficacy (Bernard (&) Goodyear, 2009; Melchert et al., 1996). Practicum is the first opportunity counselors-in-training have to apply theoretical knowledge in a professional setting, use new clinical skills, and test how well they fit into the field of counseling (O'Connell (&) Smith, 2005). Additionally, if counselor educators do not fully understand the process counselors in training develop counselor self-efficacy, they may be overlooking opportunities to educate a new generation of counselors or using their time, energy and resources in areas that may not be the most efficient in counselor development. The purpose of this study was to examine the effect of an embedded, rich-media distributed learning environment added to practicum had on the development of counselor self-efficacy, reduction of anxiety and effect on treatment outcomes for counselors in training in their first semester of practicum. This study found the use of distributed learning to extend education beyond the classroom significantly and positively affected the development of counselor self-efficacy, had mixed statistical results on the reduction of anxiety and did not have an affect on treatment outcome. Furthermore, the study used hierarchical linear modeling to see if the characteristics of individual practicums affected the three main constructs, the results did not find a significant effect from the groups.The results of the study produced several implications for counseling. First, if counselor educators help counselors in training become more aware of counselor self-efficacy, the students can better understand how the construct affects their anxiety, their comfort with expanding or improving their clinical skills and the approach they take to a client, session or treatment plan. A second implication is that using an embedded, rich-media learning environment may help the counselors in training to develop their clinical skills. The results of this study imply that utilizing technology and discussions beyond the classroom is beneficial for (a) increasing the students' counselor self-efficacy, (b) normalizing the emotions the students may experience and (c) improving the methods for development through vicarious learning. Also, as technology continues to evolve and as education continues to adapt by integrating technology into the classrooms, counselor educators should begin exploring how to best use technology to teach students during practicum. Traditionally, based on the nature of counseling, practicum has been an interpersonal experience, but the results of the current study imply the methods of extending learning beyond the traditional class time is beneficial. Finally, as counselor educators strive to increase students' counselor self-efficacy early in practicum, in an environment that contains anxiety and self-doubt (Bernard (&) Goodyear, 2009; Cashwell (&) Dooley, 2001) using vicarious learning through video and online discussions can assist in accomplishing the goal.

However, in the literature the underlying causal mechanisms for the improvement attributable to health education have not been explored, especially, how diabetes educational intervention may affect diabetes care outcomes. The purpose of the present study was to identify the causal mechanisms responsible for improved knowledge, attitude, practice and outcomes, so that educational interventions can be tailored efficiently and effectively to patients who are most likely to benefit from self-care management. The study used the knowledge, attitude, practice and outcome (KAP-O) framework. The specific purpose of the study was to examine the causes of variation in the outcomes of glycated hemoglobin (A1C), low-density lipoprotein cholesterol (LDLC), functional capacity (FC), and poor perceived health (PPH).An experimental study with a randomized control trial design involving 141 participants was conducted. The experimental group (N = 87) and control group (N = 52) were comparable in terms of demographics and major diagnoses. The experimental group received diabetes education. The control and experimental groups received usual customary care. Knowledge, attitude, practice, functional capacity and poor perceived health were measured before and after intervention using reliable and valid instruments. The study used a tailored attitudinal instrument. Glycated hemoglobin (A1C) and low-density lipoprotein cholesterol (LDLC) were measured before and after intervention. Multiple analytic strategies were applied to examine the experimental data.The four outcome variables of (A1C), (LDLC), (FC), and (PPH) did not constitute one common factor measurement model for outcome evaluation. Results of the Independent sample t-test showed that health educational intervention directly improves knowledge about diabetes. The path analysis of panel regression showed that health educational intervention directly lowers glycated hemoglobin (A1C). The causal modeling of {(Exp_Status)-(K-T2)-(A-T2)-(P-T2)} model showed that health educational intervention also indirectly improves preventive practice via knowledge. The effect of attitude (A-T2) was greater than the effect of knowledge on preventive practice of self-care. The difference-in-differences analysis showed that difference in practice (DP) statistically significantly affects the difference in glycated hemoglobin (DA1C). The greater the preventive practice, the greater the lowering of glycated hemoglobin (A1C), indicating a better control of diabetes. The data from this experiment do not support a strong causal path of experimental effects on outcomes via knowledge, attitude, and practice of self-care.The study should be replicated using the KAP-O model in research based on multi-centers, multiple providers, and a diverse population of Type 2 diabetes patients. The study should assess outcomes more than four times over a period of one to two years to elicit the trajectory of change in outcome variables. Knowledge and attitude should be assessed at baseline and continuously improved for the duration of the study.?

As part of this study, 214 culturally diverse mothers with young children who ranged in age from 2- to 6-years rated their own temperament and personality, their parenting characteristics, and their young child's functioning (i.e., temperament and emotional and behavioral functioning). When examining mothers' temperament and personality together, factor analyses revealed a three-factor solution (i.e., General Life Approach, Rhythmicity, and Sticktoitiveness) and suggested that temperament and personality generally were separate but related constructs. Hierarchical and mediation regression analyses suggested the importance of examining both temperament and personality in the context of parenting behaviors and the outcomes experienced by young children. Overall, these findings suggested that mothers' temperament and personality play a significant role in parenting young children and optimizing young child outcomes. These findings are particularly helpful for professionals working with families experiencing difficulties dealing with their young child's difficult temperament styles as well as difficult emotional and behavioral functioning.

The number of observations were compared to VAM scores to determine if teachers with a greater number of observations received higher VAM ratings. The comments recorded and submitted as feedback were also reviewed. Data were analyzed to identify relationships between the types of feedback provided to teachers and student achievement outcomes as measured by VAM scores. No significant relationship existed between VAM scores and number of observations or percentage of comments for teachers at any grade level. In addition, no significant relationship existed between predominant feedback for teachers and VAM scores.The information in this study was valuable for understanding the relationships that exist among instructional practice scores, value-added measures, and learning gains to drive conversations with teachers regarding rigorous instruction. Observations and feedback should be a tool for improvement of instruction, but the data confirmed this process continues to be compliance based with inflated scores that do not match the level of performance of students. Changing this is strongly linked to the provision of feedback associated with improving instruction and holding teachers accountable in meeting the standards outlined in the feedback. Observers are in need of professional development on how to provide effective feedback in the areas of instruction that will make the biggest impact on student achievement. Continuing to put time and effort into implementing and monitoring evaluation systems without further training and emphasis on feedback will result in the same lack of impact on student achievement outcomes and may even undermine the role of observers in providing support to teachers.

As opioid prescribing has increased over the last decade in the US, so has opioid-related deaths, surpassing car accidents and suicide as the leading cause of injury-related deaths. Medication assisted treatment (MAT) is fundamental in decreasing opioid abuse overdose and mortality. Therefore, the research study aims to determine if counseling adherence, opioid abstinence, and retention in MATs are influenced by personal characteristics, socio-economic factors, readiness to change, social support, and integrated care. Guided by social cognitive theory, transtheoretical model, and theory of reasoned action, the study will employ a retrospective cohort design utilizing opioid dependent patients from a MAT Program in West Florida. Analysis of three cox regression models indicated for personal factors: an increase in age was associated with patients being more likely to adhere to counseling (p=.001) and retention (p=.034), full-time employment (p=.043) was positively associated with opioid abstinence, whereas part-time employment (p=.037) was positively associated with retention, having insurance (public: p=.000) was positively associated with counseling adherence, opioid abstinence (public: p=.000, private: p=.035) and retention (public: p=.000, private: p=.000). With regards to environmental influences, social support was positively associated with opioid abstinence (p=.022) and integrated care was positively associated with opioid abstinence (p=.027) and retention (p=.000). Examining these factors are necessary to improve treatment adherence and expand MAT programs. Additionally, providing funding is crucial for practitioners to continually create educational intervention strategies to engage patients in treatment, thereby reducing the opioid overdose epidemic. This study extends the literature contributing to understanding personal factors and environmental influences in MATs.

The predictor variable, RN supply, and some control variables were anticipated to have an asynchronous effect on the seven outcome variables in the hypothesized relationship. Therefore, these variables were examined using three different models: contemporaneous; a three-year lagged; and a distributed lag (both contemporaneous and lagged variables). Quadratic terms for RN and physician supply variables were included. Because the Area Health Resource File (AHRF) outcome variables were skewed toward zero and left censored, Tobit regression analyses were used. Data were obtained from 19 states using historical RN Supply data for 1,472 counties, representing 47% of the total target population of 3,108 U.S. counties and county equivalents. Regions with rural populations(-)the Midwest and Southeast(-)were overrepresented. Higher RN supply is positively related to higher mortality rates from ischemic heart disease, other cardiovascular disease, and chronic lower respiratory disease in the distributed lag models. Higher RN supply is not significantly related to rates of low infant birth weight, infant mortality, or mortality from cerebrovascular disease in any model. Higher RN supply is positively related to total deaths in the contemporaneous and lagged model. The results suggest a counter-intuitive, but non-linear relationship between RN supply and health outcomes. More research is needed to understand these relationships and policies must be devised to reduce the current and growing future RN shortage.
