Show morePurpose: The threefold purpose of this research is to identify the essential antecedents of cultural competence as identified by international nurse researchers, to compare the content of the extant cultural competence instruments to these antecedents and to potentially identify gaps in their conceptualization. A secondary aim of this research is to initiate validation of Harper's model of ethical multiculturalism. Conceptual Basis: The model of ethical multiculturalism depicts the attributes of ethical multiculturalism as the fulcrum of a balance between two ethical philosophies of fundamentalism and relativism. The attributes of moral reasoning, beneficence/nonmaleficence, respect for persons and communities, and cultural competence form the pyramidal fulcrum. The antecedents form the base of the pyramid and include cultural awareness, culture knowledge, cultural sensitivity, cultural encounters, cultural skill and understanding of ethical principles. Methodology: An on-line Delphi method was conducted with 35 international nurse researchers identified through published research, university directories, and professional organizations. Consensus was reached after two rounds. Following the Delphi rounds, sixteen members of the expert panel participated in an on-line focus group to validate results of the Delphi and discuss cultural competence in the international arena. Findings: Eighty antecedents of cultural competence were identified. Focus group discussion validated findings of the Delphi. Consensual thematic analysis of the focus group transcripts resulted in six themes: chimerical, contact, contextual, collaboration, connections, and considering impact. The Transcultural Self-Efficacy Tool (TSET) contained the most antecedents identified by the expert panel. Conclusions: Cultural competence is a process, not an outcome, and must be considered from the perspective of the recipient of care or research participant. Nurses must strive to deliver culturally acceptable care. The model of ethical multiculturalism is revised to include cultural desire as an antecedent. Nurses must understand the impact of globalization on individual health and care delivery. Implications for Nursing: Further testing of cultural competence instruments is needed to determine the correlation of self-efficacy with behavior, self-assessment with client assessment, and cultural competence with client outcomes. In education, research is needed to determine the most effective methods of teaching cultural competence. Increased recruitment of minorities into nursing programs is warranted. In practice, nurses must be prepared to provide language assistance as needed, recruitment and hiring of minorities must be increased, and minority thresholds must be used to determine cultural knowledge content for organizations.

More and more improvement projects are being adopted to enhance healthcare services, making it more patient-centric, and enabling better cost control. Healthcare organizations strive to identify and carry out such improvement initiatives to sustain their businesses and gain competitive advantage. Seeking to reach a higher operational level of excellence, healthcare organizations utilize business excellence criteria to conduct assessment and identify organizational strengths and weaknesses. However, while such assessments routinely identify numerous areas for potential improvement, it is not feasible to conduct all improvement projects simultaneously due to limitations in time, capital, and personnel, as well as conflict with other organization's projects or strategic objectives. An effective prioritization and selection approach is valuable in that it can assist the organization to optimize its available resources and outcomes. This study attempts to enable such an approach by developing a framework to prioritize improvement opportunities in healthcare in the context of the business excellence model through the integration of the Fuzzy Delphi Method and Fuzzy Interface System. To carry out the evaluation process, the framework consists of two phases. The first phase utilizes Fuzzy Delphi Method to identify the most significant factors that should be considered in healthcare for electing the improvement projects. The FDM is employed to handle the subjectivity of human assessment. The research identifies potential factors for evaluating projects, then utilizes FDM to capture expertise knowledge. The first round in FDM is intended to validate the identified list of factors from experts; which includes collecting additional factors from experts that the literature might have overlooked. When an acceptable level of consensus has been reached, a second round is conducted to obtain experts' and other related stakeholders' opinions on the appropriate weight of each factor's importance. Finally, FDM analyses eliminate or retain the criteria to produce a final list of critical factors to select improvement projects. The second phase in the framework attempts to prioritize improvement initiatives using the Hierarchical Fuzzy Interface System. The Fuzzy Interface System combines the experts' ratings for each improvement opportunity with respect to the factors deemed critical to compute the priority index. In the process of calculating the priority index, the framework allows the estimation of other intermediate indices including: social, financial impact, strategical, operational feasibility, and managerial indices. These indices bring an insight into the improvement opportunities with respect to each framework's dimensions. The framework allows for a reduction of the bias in the assessment by developing a knowledge based on the perspectives of multiple experts.

In the United States, the vast majority of clients consider themselves to be spiritual or religious, so counselors who lack competence in addressing spiritual and religious issues in counseling are likely to offer ineffective or perhaps unethical care to clients. Counselor educators must improve education and assessment in this critical specialty area of counseling. Of primary concern is a student's ability to demonstrate spiritual competence in counseling. The 2009 ASERVIC Spiritual Competencies offer the most comprehensive standard of spiritual competence in counseling in any mental health profession, however there is no reliable and standardized assessment that measures demonstrated spiritual competency. Competency can best be measured when the examinee makes choices in a context that is similar or the same as that in which he or she will practice, therefore an effective competency measurement must include client cases. The purpose of this study was to investigate whether a case based assessment for measuring clinical judgment in situations of uncertainty, called a Script Concordance Test, could be constructed by experts using the Delphi Method. This instrument was based on the 2009 ASERVIC Spiritual Competencies as the standard for demonstrated competence. The results of this study indicated that expert practitioners and educators could come to consensus on appropriate cases, appropriate competencies to measure in each case, items to assess competency in each case, and an instrument that included items assessing all 14 of the 2009 Spiritual Competencies. Additionally, the constructed instrument demonstrated excellent test retest reliability and adequate internal reliability.There are several implications for counselor education. First, this study provides evidence that expert practitioners and educators can come to consensus to construct a highly contextual instrument to measures clinical decision making about spiritual competence in counseling. Second, a promising new type of instrument with excellent reliability and strong content validity has been introduced to the field of counselor education. Third, with appropriate assessment, counselor education programs can begin to measure student competence, in terms of clinical judgment, on addressing spiritual and religions issues in counseling over time because this instrument is appropriate for use at different intervals throughout professional development. Fourth, the format of this instrument is also useful for educational purposes and reflective practice. Finally, the theoretical foundations of the Delphi Method and script concordance tests are compatible with one another and with instrument development. The researcher recommends that future studies to construct script concordance tests for other specialty areas of competence employ and refine this method.

This dissertation was developed to address the need to use feedback from multiple stakeholder groups to define quality and assess an entity's efficiency at achieving such quality.A decision support model with applicability to diverse domains was introduced to outline the approach. Three phases, (1) quality model development, (2) input-output selection and (3) relative efficiency assessment, captured the essence of the process which also delineates the approach per tool applied. This decision support model was adapted in higher education to assess academic departmental efficiency at achieving stakeholder-relative quality. Phase 1 was accomplished through a three round, Delphi-like study which involved user group refinement. Those results were compared to the criteria of an engineering accreditation body (ABET) to support the model's validity to capture quality in the College of Engineering (&) Computer Science, its departments and programs. In Phase 2 the Analytic Hierarchy Process (AHP) was applied to the validated model to quantify the perspective of students, administrators, faculty and employers (SAFE). Using the composite preferences for the collective group (n=74), the model was limited to the top 7 attributes which accounted for about 55% of total preferences. Data corresponding to the resulting variables, referred to as key performance indicators, was collected using various information sources and infused in the data envelopment analysis (DEA) methodology (Phase 3). This process revealed both efficient and inefficient departments while offering transparency of opportunities to maximize quality outputs. Findings validate the potential of the Delphi-like, analytic hierarchical, data envelopment analysis approach for administrative decision-making in higher education. However, the availability of more meaningful metrics and data is required to adapt the model for decision making purposes. Several recommendations were included to improve the usability of the decision support model and future research opportunities were identified to extend the analyses inherent and apply the model to alternative areas.

To better prepare teachers, the researcher conducted a two-phase study, situated in the framework of the Skill Acquisition Model (Dreyfus (&) Dreyfus, 1986) to explore the behaviors of novice and expert teachers in a simulated secondary inclusive environment. This classroom included a virtual student with autism. In phase one, the researcher conducted a Delphi Study to determine the best practices, perceived by experts in the field, for teachers who serve students with ASD in inclusive secondary environments. During phase two, the researcher used the list of skills identified as a framework to observe and interview 10 teachers, five novices and five experts, in a simulated secondary inclusive environment. The researcher identified 11 high leverage simulation practices (HLSP) that expert teachers should use while teaching in a simulated secondary inclusive environment. Observations and reflections of expert and novice teachers were analyzed, finding only 4 HLSP among experts and 5 HLSP among novice teachers. Additional HLSP were seen through the teachers' reflections. Data were analyzed and discussed in detail. Implications for practice and recommendations for future research in teacher preparation is provided.
