Previous research has identified some specific group differences in neurocognitive functioning between individuals diagnosed with social phobia and nonpsychiatric controls, but has failed to administer a comprehensive neuropsychological battery to a social phobia patient group, resulting in a piecemeal understanding of the neurocognitive functioning of this population and an incomplete picture of the neuropsychological profile inherent to this group. The present research utilized a broader collection of neuropsychological tests to assess nine cognitive domains: Verbal Learning, Verbal Delayed Memory, Visual Immediate Memory, Visual Delayed Memory, Visual-Spatial Processing, Verbal Working Memory, Visual Working Memory, Executive Functioning, and Attention. A mixed analysis of variance (ANOVA) did not reveal a significant group by cognitive domain interaction, nor a significant main effect of group. As this was the first study to examine multiple cognitive domains in a single sample of individuals with generalized social phobia, exploratory univariate analyses were performed to examine group differences for the specific cognitive domains. This revealed significant group differences specific to the Visual Working Memory domain, with the social phobia group scoring significantly lower than the nonpsychiatric control group. Implications of these findings and directions for future research are discussed.

Show moreAn evaluation of executive functioning is a critical component of a comprehensive assessment of higher cerebral functioning. The Tinker Toy Test (TTT) was introduced in 1982. This test allows an individual to demonstrate the extent of their executive capacities by permitting them to initiate, plan, and structure a potentially complex activity and carry it out independently in an unstructured fashion and administration is simple. This is a departure from more complex and structured tests of executive function. There is a dearth of research on the TTT and this study seeks to examine some of the psychometric properties of this instrument; i.e., working time minimum, gender effects, convergent and divergent validity, and potential intellectual correlates. Participants included 10 male and 30 female student volunteers from a large university in Central Florida. Participants had no history of neurologic disease/trauma or conditions that would affect motor functioning of the upper extremities. Participants completed a demographic questionnaire, the WASI-II, and the TTT. A two-way mixed-design ANOVA examining TTT scores as a function of work time and gender revealed a non-significant gender main effect, F(1, 21) = .09, p = .767. The work time main effect was not significant, , F(1, 21) = .324, p = .575. A significant work time x gender interaction was observed, F(1, 21) = 4.983, p = .037. Convergent validity was assessed by comparing the TTT scores with the Matrix Reasoning subtest, r(38) = .32, p = .044, and the Similarities, r(38) = .34, p = .03, subtest on the WASI-II. Divergent validity was assessed by comparing TTT scores to the Block Design subtest of the WASI-II, r(38) = .245, p = .127. No significant correlation was found between intelligence and TTT (VCI, r(38) = -.16, p = .335; PRI, r(38) = .15, p = .344; and FSIQ, r(38) = -.02, p = .928). The data supports the continued use of the 5-minute working time minimum presented by Lezak, as this temporal index was a more accurate representation of executive functioning. This study demonstrated no association between TTT scores and intellectual functioning. The findings of this study support the validity of this underutilized test of executive functioning and its inclusion in neuropsychological test batteries.

Neuropsychology has also begun to assimilate into primary care, as neurocognitive impairment is a correlate of many medical disorders. Subjective cognitive decline (SCD), a common complaint among older adults, is an increasingly recognized warning sign of non-normative cognitive aging. These patients typically present first to their primary care providers who may play a critical role in the early detection of cognitive impairment. Given the growing awareness about cognitive health and disability, the importance of neuropsychological assessment as a standard component of integrated care has been recognized by providers. Thus, the purpose of this study is to examine the efficacy and feasibility of neuropsychological services, for memory concerns, in a community primary care setting. The study also explored the relationship between SCD and performance on neurocognitive measures and satisfaction levels for both patient participants and medical providers. A total of 16 patient participants completed the study. On average, patients were in their late-60's and mostly female and Caucasian. Participants completed a brief interview, neurocognitive evaluation, self-report measures of SCD and mood, and satisfaction survey. Results did not reveal significant correlations between SCD and neurocognitive performance. Significantly more referrals were made to the onsite neurocognitive clinic, than were made for outside services in a nine-month period preceding the described program. Patients referred to the onsite clinic were also significantly more likely to have an accessible report located in their EMR than those referred offsite. Both participants and medical providers were reportedly satisfied with clinic services. Results suggest that a clinic of this nature has promising benefits and is well-liked by both patients and providers, though barriers related to full utilization of services remain a challenge. Further research with a larger, more diverse sample is recommended. ?

All participants had no history of neurologic disease/trauma, or conditions that would affect motor functioning of the right and left upper extremities. Data were analyzed using a 3-way mixed-design ANOVA. Results revealed a significant main effects for gender (F(1, 59) = 5.215, p = .026) and handedness (F(1, 59) = 6.362, p = .014). Of primary interest was the main effect for placement direction, which was not significant, F(1, 59) = .120, p = .731. No significant interaction was observed (all p > .40). Recommendations for the use of this test in applied neuropsychological settings are offered.

In addition to general commissioning requirements and aeromedical standards, the U.S. Navy utilizes the Aviation Selection Test Battery (ASTB) to select commissioned aviation students. Although the ASTB has been a good predictor of aviation student performance in training, it was proposed that incremental improvement could be gained with the introduction of novel, computer administered performance-based measures: Block Rotation (BRT) and a Navy-developed Compensatory Tracking task. This work constituted an initial validation of the BRT, an interactive virtual analog of Shepard-Metzler's (1971) Mental Rotation task that was developed with the intention of quantifying mental rotation and psychomotor ability. For Compensatory Tracking, this work sought to determine if data gathered concord with results in extant literature, confirming the validity of the task. Data from the BRT were examined to determine task reliability and to formulate relevant quantitative/predictive performance human models. Results showed that the BRT performance is a valid spatial ability predictor whose output can be modeled, and that Compensatory Tracking task data concord with the psychometric properties of tracking tasks that have been previously presented in the literature.

Show moreAnxiety disorders are among the most commonly diagnosed class of mental illness in the United States, and often involve abnormally high levels of stress and social fear. Despite high lifetime prevalence rates, social anxiety disorder (SAD) has remarkably low diagnosis and treatment rates. Furthermore, while individuals with other specific psychiatric disorders tend to exhibit significant neuropsychological deficits, neuropsychological functioning in individuals with SAD remains largely untested. A majority of the few existing studies concerning neuropsychological performance in SAD samples focus on specific functions, and their limited results are highly mixed. The primary objective of this investigation was to provide a more thorough, broad assessment of both auditory and visual working memory as related to psychometrically-defined social anxiety disorder. In addition, this study aimed to help clarify as to whether such deficits are related to the construct of social anxiety, or whether any potential deficits are better explained by generalized state and/or trait (in-the-moment) anxiety. The implications of a deficit in the visual and/or auditory working memory domains are multifaceted. For example, such a deficit may lead to the inability to detect visual cues in social situations. The inability to process these social cues has the potential to exacerbate some SAD- related symptoms, such as fear of humiliation and judgment. Twenty-nine college students completed both phases of this study, including an assessment of state and trait anxiety as well as social phobia and a four-part working memory battery. An analysis of the Phase II data indicates that individual scores on the four measures of both visual and auditory working memory did not relate to trait and/or state anxiety or psychometrically-defined social anxiety. Thus, it appears that social, generalized trait, and generalized state anxiety do not relate to a neuropsychological deficit in either type of working memory in this sample population. However, we did find a statistical trend suggesting that as social anxiety increased, there was a relative decrease in visual vs. auditory working memory. This statistical trend remained after covarying for state and trait anxiety respectively. Therefore, future research in this area should examine the discrepancy in performance between the auditory and visual working memory domains as it relates to both diagnosed social phobia and psychometrically-defined social anxiety.
