Show moreCancer, an aggressive chronic disease, impacts the lives of millions of people every day. There are numerous proposed triggers related to the diagnosis and much remains to be learned about the causes. Associated with this disease's variability is the challenge to identify a single causative agent that lead to its prevention. Specific topics that need additional evidence relate to environmental factors and lifestyle behaviors in the development, treatment and, in some instances, even suppression of disease progression. Specifically, a diet that excludes animal-based products but consists of a variety of fruits and vegetables (i.e., plant-based vegan diet) is reported to retard disease progression among some individuals diagnosed with cancer. The purpose of this thesis was to examine the effects of a plant-based vegan diet as a treatment approach with individuals who have been diagnosed with cancer. The methodology included a systematic review of literature focusing on use of a plant-based vegan diet on cancer risks in research articles published in peer reviewed journals from 2006 to 2018. Consistent findings include evidence that a diet high in fruits, vegetables, whole grains, and minimal amounts of animal products showed a decrease risk of the development of various types of cancer. Recommendations for nursing education, practice, policy and research are discussed.

Show moreThe purpose of this integrative literature review was to understand the effects of medical cannabis for chronic pain management in adults. Anecdotal reports suggest the use of medical marijuana as a pain management therapy could be an alternative to opioids and other medications which have long term consequences. Potential uses span the health care continuum, from prescribed outpatient symptom management, to acute care, extended care, home care, and hospice treatment settings. The methodology included a review and synthesis of relevant research articles from 2012 to 2018, written in the English language. The findings suggest medical cannabis has the potential of effectively managing chronic pain in older adults. Adverse effects, if present, are mild and resolve without intervention. Lower doses of medical cannabis were reported to be more effective in treating chronic pain compared to higher doses. Inconsistencies in the efficacy of THC were noted compared to CBD for managing neuropathic pain. Implication for nursing practice, policy, education, and recommendation for future research were discussed along with study limitations.

Show moreThe purpose of this integrated review of the literature was to explore the postsurgical infection complications commonly occurring in individuals undergoing total knee replacement surgery, and the relationship of co-morbidities, lifestyle choices, and genetics on the risk for complication. A comprehensive search of the literature focusing on the patient surgical site infection and total knee replacement surgery using the Cumulative Index to Nursing and Allied Health Literature (CINAHL) and Medical Literature On-line (MEDLINE). Initial searches revealed 80 results. Upon closer scrutiny, duplicates were removed, as well as those not relevant to infection in total knee arthroplasty. This resulted in a review of 6 articles that fit the inclusion criteria. Inclusion criteria were articles written in the English language and published in scholarly, peer-reviewed, journals from 2009 to present. In addition to published articles, pertinent material from current nursing textbooks was evaluated and chosen to further substantiate the literature results. In cases of knee arthroplasty, diagnoses of obesity and diabetes were discovered to be risk factors for postsurgical infection. The findings of this thesis offer interpretation for nursing practice, research, education, and policy. Implications for nursing research, policy, education and practice are highlighted along with limitations of this integrative review.

Show moreThe purpose of this literature review was to identify differentiating risk factors in women for postpartum depression and postpartum psychosis. By understanding differentiating risk factors health care professionals, and nurses in particular, can be alert to women who are at higher risk for postpartum depression or postpartum psychosis. This information allows for early nursing intervention and the development of appropriate treatment plans. Fifteen peer-reviewed, English language research articles published between 2000 and 2015 were analyzed for the purpose of this literature review. Study results were inconclusive for the intention of this review, but do provide valuable information on independent risk factors for both disorders. A history of depression and significant life stressors are strong predictors for postpartum depression. Whereas a history of bipolar disorder is strongly associated with the development of postpartum psychosis. Further research is needed to examine the role of genetics in both postpartum depression and postpartum psychosis and to further evaluate risk factors for postpartum psychosis, specifically in women with no history of bipolar disorder. Moreover, additional research needs to be conducted within the United States due to a lack of generalizability of studies conducted in other nations.

Show moreThe purpose of this integrative review of the literature is to examine the effectiveness of equine assisted therapy with individuals diagnosed with Autism Spectrum Disorder (ASD). An extensive review of the literature was conducted regarding children diagnosed with ASD who received equine therapy, equine psychotherapy, therapeutic horseback riding, or horse therapy. Inclusion criteria include articles written in the English language, articles that were peer-reviewed, and had a publication date from 2006 to present. Fourteen articles were analyzed regarding children with ASD who participated in different types of equine therapy. The majority of the studies found individuals with ASD who participated in equine therapy had improvements in behavior, social interactions, and communication with decreased stereotypical ASD behaviors. Only one article found no changes in behavior. Most of the articles had very small sample sizes that limited the generalizability of the findings. There were many different ways that the research was conducted and therefore the screening measures were completely different for all studies. Many studies did not take gender of participants into account, whether the participants were verbal or nonverbal, and other treatments the participants were receiving. Although the literature analyzed had different methodologies, the overall research suggests equine assisted therapy can be a beneficial therapy for individuals with ASD and therefore could be considered a viable treatment option. The outcomes of this integrative review supports the need for larger, randomized, controlled trials with larger samples to effectively evaluate the effects of equine therapy.

Peer-reviewed journals were retrieved from MEDLINE, CINAHL, CINAHL Plus with Full Text, Cochrane Central Register of Controlled Trials, Music Index to evaluate their relevance to music therapy use in people diagnosed with dementia. English and Spanish-language research articles published from 2000-2017 were included for the review (n=15). The literature indicates the use of music therapy as an adjuvant therapy for the treatment of dementia in older adults may be beneficial in decreasing symptoms of aggression, anxiety, agitation and depression. Music therapy is used in varying settings, but more specifically in long term care facilities. This complementary therapy enhances the quality of life in persons with dementia and facilitates empathetic relationships between residents and the staff.The literature indicates that music therapy, involving singing, listening to music, music and movement, when performed by a certified music therapist can have beneficial effects for people with dementia, especially when pharmacological treatments alone do manage symptoms associated with this disorder. Implications for nursing practice, education, policy and research are discussed along with study limitations.

Show moreTraditionally, the lithotomy position is the most commonly used birthing position in western cultures. The purpose of this literature review was to examine the differences in maternal and neonatal outcomes when alternative labor positions are used. Implications for future use of alternative labor positions were explored. Peer reviewed, English-language research articles published from 2009 to 2015 were included for synthesis. Study results revealed that the lithotomy position is linked to multiple negative maternal outcomes. Birthing in a water pool or in a lateral position has been linked to more optimal maternal and neonatal outcomes compared to other positions. Health care providers, and nurses in particular, should be educated on the benefits and consequences of both the lithotomy position and alternative labor positions. Using this evidenced based research, health care professionals can educate women and families as they encounter the birthing process. Further research is needed to identify additional birthing outcomes of labor positions, specifically neonatal outcomes of the lithotomy position. In addition, research on innovative obstetric monitoring techniques will expand the opportunity to use alternative labor positions while protecting the mother and fetus.

This integrated literature review examined birthing experiences from the maternal perspective and focused on the short-term and long-term implications of negatively perceived maternal experiences. While there are several international studies, only a few have been published studies from the United States. The integrated review showed a significant impact of provider-client communication and shared decision making on a woman's overall perception of her birthing experience. Reports of excess intervention, lack of informed consent, and a perceived disregard for embodied knowledge- all contribute to negative perception of the birthing women. Implications for nursing education, practice, policy, and research are discussed in depth with a focus on improving maternal perception of the birth experience.

Show moreThe lesbian, gay, bisexual, and transgender (LGBT) community is a unique population that has specific health issues and health care needs associated with lifestyle behaviors that increase risk for certain diseases. Health concerns include mental and behavioral health, issues associated with gender identity and relationships (i.e. intimate partner violence), sexually transmitted infections, and chronic illnesses. The research suggests poorer health outcomes for the LGBT population compared to heterosexual and/or cisgender counterparts. Most research conducted with LGBT populations occurred in more populated urban settings with very few studies focusing on this population in the rural context. Consequently, there is a paucity of information on the health care concerns of the rural LGBT population. Considering the information gap, this integrative review of 14 research articles focused on health-related issues of the LGBT population in rural regions. The findings revealed rural LGBT persons experience disparities in accessing health care and support services; coupled with health care providers who often were not culturally competent; and, sometimes, unfamiliar with evidence-based health care protocols when caring for the LGBT patient. Implications for nursing research, education, practice, and policy and study limitations are highlighted.

Show moreBurn wounds tend to be a critical problem with a complicated healing process. Although advancements have been made and the treatment of burn wounds has improved significantly, the healing process for deep-partial thickness burn wounds remains problematic. The purpose of this thesis is to review the available literature on an innovate biotechnology, autologous skin cell-spray transplantation, to more effectively treat burn wounds and potentially other injuries in the future. This study was conducted by critically researching and comparing (N=7) peer-reviewed research articles focusing not only on burn wounds using traditional treatments, but also the treatment of burn wounds using revolutionary cell-spray autographing technologies. The findings in this thesis show significant enhancement using this innovative approach for the treatment of burn injuries, and presents pivotal information for future nursing research, clinical practice as well as policy and education.

Show moreRegardless of the setting, medication errors are of great concern when associated with an individual's health outcomes, along with the increased costs to society, healthcare institutions, and providers. Current research focuses on medication error data primarily in acute and extended care facilities. However, there is a paucity of research examining the causes of medication errors that occur post hospital discharge when individuals transition to the home. The purpose of this integrative literature review is to examine risk factors for medication errors outside of these settings, specifically in the home. A systematic literature search was conducted using multiple databases for relevant articles in the English language between 2006 to 2017, including CINAHL, MEDLINE, PubMed, and PsycINFO. Search terms included 'medication errors', 'home care', 'post-discharge', 'hospital readmission', and 'medication error risks in the home'. Exclusion criteria included medication errors in acute and extended care settings. The integrative review involved reading, analyzing and selecting articles, and summarizing on a matrix. Findings on occurrences of medication errors in the home included impaired client mental status, confusion related to medication names, limited understanding of medication purpose in the care plan and its side effects, level of health literacy, and client-provider miscommunication in discharge planning. Consistent and conflicting findings are discussed along with gaps in the literature. Limitations and implications for nursing practice, policy, research, and education are also noted.

The purpose of this integrative review is to examine the use of AAT focusing on canine assisted therapy (CAT) among veterans diagnosed with PTSD. The methodology involved database searches, including MEDLINE, PubMed, PsychInfo, EBSCOhost, along with textbooks and popular media published from 2000 to 2016. Associated with the lack of more recent research, relevant articles published before 2000 were included in the review. Search terms included, 'veterans,' 'service dogs,' 'service animals,' 'animal-assisted therapy,' 'canine therapy,' 'PTSD,' 'post-traumatic stress disorder,' 'psychiatric,' 'U.S. veterans,' 'equine therapy,' 'horse therapy,' 'pet therapy,' and 'military veterans.' A total of ten relevant studies were identified which focused on the use of AAT among veterans diagnosed with PTSD. Different populations diagnosed with PTSD and other behavioral and psychiatric health conditions using AAT were examined as well. These articles were read, analyzed, and synthesized. Results of the review offer some support that AAT has psychological, physiological and psychosocial benefits for some populations across the lifespan with various diagnoses. Consistent and conflicting findings along with gaps in the literature are highlighted. Limitations and implications for nursing practice, research, policy and education also are noted in this thesis.

The reviewed articles were published between the years of 2000 to 2014, written in the English language, scholarly in nature, in peer-reviewed journals with access to full text electronic versions of the article, along with articles that provided current anecdotal information related to the effects of AAT. Search terms included the phrase, "animal assisted therapy" in the title. Results of the literature search yielded an enormous quantity of recent and innovative research on various aspects of AAT. Various animals were identified with ATT, most often canines; however, it is not uncommon to find that horses, dolphins, felines, birds, or even small mammals satisfying the same roles. Of these, for this thesis thirteen research articles were selected that dealt primarily with canines in AAT. The findings from the review for this thesis suggested that despite the large volume of available research on ATT in general, a gap in literature exists focusing on the health outcomes with specific animal species. Additionally, the research was noted to be fragmented in nature and the findings were inconsistent; thus, limiting the development of evidence based ATT interventions. Implications for nursing research, policy, education and practice are discussed, along with limitations of this integrative review.

This study had two purposes: 1) to explore cultural beliefs regarding health-care seeking behaviors in Puerto Ricans with diabetes who live in South Florida; and 2) to examine Puerto Ricans' perceptions about their health-care providers. The cultural phenomena of interest were familism, religiosity, spirituality, use of ethnomedicine, and perception of ethnic concordance of health-care provider. Numerous studies have examined these phenomena with other Latino groups, but none have specifically focused on Puerto Ricans. An overarching goal of the study was to contribute to the knowledge base on a particular health disparity--diabetes. Using a narrative inquiry approach, a purposive sample of self-identified Puerto Ricans with diabetes (N = 12) were recruited from six sites in a South Florida city. Data were obtained using a pencil-and-paper demographic instrument, the Short Acculturation Scale to determine language preference of Spanish or English, and a personal interview using a semi-structured, ten-item interview guide. Subjects gave written informed consent for participation, and all data were coded to ensure confidentiality. The personal interviews were tape recorded and transcribed verbatim. Interviews completed in Spanish were translated to English and transcribed. Using content analyses techniques, transcribed narratives were analyzed for content and thematic emergence. iii The findings revealed that familism was an important consideration in health-care seeking behaviors. Traditional gender role expectations, coupled with caregiver burdens, deterred some participants from seeking care even when care was needed. Religiosity and spirituality did not influence decision-making but did have a role in coping with the chronic disease. While participants were aware of culturally based ethnomedicine, they preferred Western medicine for the treatment of their diabetes. They also had a preference for a health-care provider who was ethnically concordant. Serendipitous findings that emerged in the analyses included the casual attitude of several participants about their diabetic status and reports of depressive-like symptoms among most of the women. Health-care providers need to take into consideration the cultural and linguistic preferences of Puerto Ricans to develop an appropriate and effective treatment plan. Discrepancies between the health-care providers and the clients' systems must be reconciled to improve adherence to evidence-based treatment.

The philosophical underpinnings for this study were based on the phenomenological method of bracketing which allows the researcher and subjects to focus on lived experience. This qualitative methodology provided a means to examine the phenomenon of interest in depth from the participantsÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂ' subjective perspective. Data were collected through face-to-face interviews and follow up telephone conversations. GiorgiÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂÃÂ's method for analyzing phenomenological data was used to elicit an invariant description of the meaning that creating art had for the participants. Three predominate themes emerged from the analysis. Contextual constituents of the phenomenon were identified as that of giving back and time to create. Dynamic components consisted of creative space and creative expression. Enduring factors consisted of the creative experience and sense of self. Nurses are in a unique position to facilitate the creative art process which holds the potential for self-healing and self-responsibility for their patients. Included are limitations of the study and recommendations for future research.

Poorly understood and defined, based on culture, and expensive for any healthcare system, PNC remains a target for change and improvement. The purpose of this cross-national qualitative research study using narrative inquiry methods was to explore consumer perspectives of individual health and routine PNC in the USA and Iceland. Methods: A purposive sampling technique was used to identify study participants (n = 32) from the United States (n = 16) and Iceland (n = 16). Data were collected via a semi structured interview which included demographic questions. Results: Content analysis processes were used to analyze the transcribed narratives to identify common conceptual themes. Subsequently, the narratives of the women from the two nations were compared to identify cultural variations about PNC. In respect to the findings, demographically the two groups were similar. Respondents from both nations preferred a more supportive role from PNC providers with adequate time to explore important personal concerns and less emphasis on monitoring weight gain during the pregnancy. Variations between the two groups related to the health care delivery system of the nation in which respondents resided. These findings have the potential for expanding the definition of PNC to include consumer perspectives. Discussion/Implication: Additional research is needed with other groups of women to validate, clarify and expand identified themes, as they may improve PNC, and ultimately, perinatal outcomes. Improving PNC holds the promise of improving infant mortality while at the same time reducing healthcare expenditures for countries around the world.

Show moreThis qualitative study included women who had gone through the menopausal transition and had experienced obesity, and it focused on their weight histories and experiences across the life course. The goal of this research was to add to the body of knowledge concerning weight gain by applying a novel middle range theory (story theory). Story theory was used to collect and interpret from women's life course stories the critical themes and patterns of their weight gain. Oral accounts were elicited during personal interviews from a convenience sample of ten women recruited from a weight loss and exercise program in Central Florida. Literature focusing on the prevalence of obesity, contributing factors and associated complications, as well as treatment approaches is extensive. A variety of approaches have been proposed to identify factors that contribute to the development of obesity across the lifespan. Ultimately, the goal of these studies is to understand risk factors for weight gain along with corresponding prevention and management strategies. A particular life course approach focuses on critical periods across the life span that may be associated with risk for the development of obesity. For women, puberty, pregnancy and menopause are noted to be critical for weight change in the life course as they are associated with hormonal changes and changes in body composition including fat mass. Story theory was chosen to conceptualize and guide participants through a personal interview in order to share their weight experiences along their life course. Content analysis procedures were used to analyze the data in order to identify themes and corresponding verbatim exemplars. A re-constructed composite story was developed that included excerpts from the participants' stories in order to reveal contextualized results. Themes that were identified relative to participants' experiences with their weight included: changes associated with emotional and physical health; eating patterns associated with multiple and/or changing roles/relationships; and, changes in the environment. An interpretation of the predominant pattern of weight gain included: changes in eating and physical activity that occur during multiple and simultaneous transitional life experiences, primarily in adulthood. The findings suggest that transitional experiences in women's lives - physiological, developmental, relational or environmental - were critical in that they presented risk for behavior changes related to eating and physical activity. The results of this study and the use of story theory have implications for providing individualized, patient-centered lifestyle recommendations for the prevention of unhealthy weight gain.

Show moreAssociated with health care reform, there has been a growing interest among healthcare professionals regarding use of energy-based therapies as a complementary therapeutic intervention. The purpose of this integrative literature review was to examine the impact of energy-based therapies (i.e. healing touch, reiki, therapeutic touch) on patients� postoperative recovery. The methodology included identifying appropriate peer-reviewed, English-language research articles on the topic area that were published between 2006 to 2018. Following a search in select data bases, articles focusing on the topic were critiqued, analyzed and synthesized by the researcher. Consistent and inconsistent findings along with gaps in the literature are noted. In general, the research supported positive postoperative recovery outcomes when energy-based therapies were included along with standard postoperative nursing care. This theses highlights implication for nursing practice, education, and policy and identifies study limitations. Given that research is limited on this topic area, additional studies are needed to establish supporting evidence to ascertain the effects of energy-based therapies when used as a complementary postoperative intervention.
