The Consensus Auditory Perception Evaluation of Voice (CAPE-V) was used to asses 1.) overall severity, 2.) roughness, 3.) breathiness, 4.) strain, 5.) pitch and 6.) loudness from a sample of pediatric voices. Twenty-four graduate clinicians who had completed a graduate level course in voice disorders participated in the study. Twelve of the participants were randomly selected to complete a perceptual training course prior to the evaluation session. Voice samples included 10 disordered and 2 normal voices from a population of children age 3-10 years old. The 12 voice samples were randomly repeated 3 times. Results of analysis of variance indicated that the groups significantly differed in their severity rating of the perceptual indices, suggesting that training affected the participants' judgment of severity. Additionally, variability was reduced as a function of training. Pearson product-moment correlation coefficient's revealed a moderate to strong relationship for all of the perceptual indices suggesting that regardless of training participants have an implicit understanding of normal versus disordered voice samples.

Show moreThe purpose of this study was to investigate the effects of voice production and perception of dyspnea in aerobic instructors during simultaneous tasks of exercise and speech production. The study aimed to document changes that occur during four conditions: 1) voice production without exercise and no use of amplification; 2) voice production without exercise and the use of amplification; 3) voice production during exercise without the use of amplification; 4) voice production during exercise with the use of amplification. Participants included ten aerobic instructors (two male and eight female). The dependent variables included vocal intensity, average fundamental frequency (F0), noise-to-harmonic ratio (NHR), jitter percent (jitt %), shimmer percent (shim %), and participants' self-perception of dyspnea. The results indicated that speech alone, whether it was with or without amplification, had no effect on the sensation of dyspnea. However, when combining speech with exercise, the speech task became increasingly difficult, even more so without the use of amplification. Exercise was observed to inhibit vocal loudness levels as vocal intensity measures were lowest in the conditions with exercise with the use of amplification. Increases in F0 occurred in conditions involving exercise without the use of amplification. Moreover, four participants in various conditions exhibited frequencies that diverged from their gender's normal range. Participants' NHR increased during periods of exercise, however no participants were found to have NHR measures outside the normal range. Four participants were found to have moderate laryngeal pathology that was hemorrhagic in nature. Findings suggest that traditional treatment protocols may need to be modified beyond hygienic approaches in order to address both the respiratory and laryngeal work-loads that are encountered in this population and others involving similar occupational tasks.

Show moreThe purpose of the present study is to test the feasibility of implementing a video-game based intervention protocol as a means to improve therapy compliance in school age children with hyperfunctional voice disorders. Three levels of modification were made to an existing entertainment software program in order to implement the therapeutic protocol and test compatibility. The third level of modification included a two-phase quasi-experimental single subject design with a school age participant receiving the video game therapy protocol and traditional therapy for equal time. The independent variables for this study included the mode of voice therapy delivery (traditional vs. video game). The dependent variables included therapy compliance, perceptual evaluations and acoustic measures. This study found that a purely entertainment video game can be implemented as a therapeutic protocol for a school age child diagnosed with a vocal pathology. Results illustrated no change in compliance with non-traditional therapy versus traditional therapy. However, perceptual measures improved post treatment for breathiness, strain and overall severity, as well as significant differences for mean amplitude. Discussion will focus on implications of employing video game based therapy and design of future studies.

Show moreLaryngopharyngeal reflux (LPR) is currently one of the most prevalent conditions associated with voice disorders being treated in voice care centers worldwide. Many singers experience voice related disturbances but are unaware that these disturbances may be the result of LPR. The purpose of this study was to quantify the perceptual symptoms and objective measures of LPR in a population of singers in order to understand the relationship between perceived symptoms, laryngeal findings, and evidence of acid exposure to the larynx. The Reflux Symptom Index (RSI), Reflux Finding Score (RFS), and the Dx-pH monitoring system were used to quantify participant symptoms, endoscopic findings, and pH levels in the oropharynx. The population included 12 semi-professional and professional singers. Significant correlations were found between the RFS, RSI and pH mild and moderate pH levels. This indicates that singers are sensitive to even small deviations of pH and this should be taken into consideration when evaluating a singers who have suspected LPR. Due to the variety of etiologies that can produce the symptoms and physical findings mentioned in this study, it is imperative that more objective data be obtained to confirm the presence of reflux in the oropharynx. As such, endoscopic findings and symptoms alone are not a good indication of reflux exposure and more objective data, like an oropharyngeal pH measurement system, should be implemented to quantify reflux in the oropharynx. The RSI and the RFS are valid tools for qualifying perceptions and physical findings however they are not without flaws.

Show moreResearch in Speech-Language Pathology provides the basis for understanding how and to what degree therapeutic techniques and interventions affect the health and quality of life of individuals with communication disorders. Across numerous fields of healthcare, research serves just as important a function and is used to guide the practices of professionals across the public health sector. Several disciplines, including chiropracty, nursing, and physical therapy, rely on the implementation of Evidence-Based Practice (EBP) to ensure that the process of rehabilitation and the techniques employed therein are carried out though the synthesis of best clinical judgment, empirical evidence, and patient values. The field of Speech-Language Pathology also uses EBP as the foundation of intervention and rehabilitation. Research has shown, however, that clinicians in a variety of settings encounter barriers to the implementation of EBP. Such reported barriers include lack of access to current research literature, lack of time with which to review the literature, and difficulty determining the quality of research available. General aims of the present study were: (1) to explore speech pathologists' self-reported patterns of access and use of techniques presented in the current research literature, (2) to investigate their self-rated knowledge of and ability to critically evaluate the research literature, and (3) to examine their knowledge and ability through use of a problem-based survey design. 325 certified Speech-Language Pathologists (SLPs) were administered a brief web-based survey to collect information that might provide insight related to these research questions. Results of the study indicated that, on average, participants access the research literature and utilize techniques found therein with moderate frequency. Findings also reveal that self-rated capacity and research knowledge were significantly greater for respondents who had acquired their PhD and for those who had previously conducted formal research. Significant results were also found when evaluating differences among respondent's place of work by median change in self-rated research capacity. Furthermore, positive correlations were found between research knowledge and self-rated capacity and research knowledge and research evaluation. These findings call attention to the process through which research methodology is taught in higher education. The findings may also suggest that a more effective and functional model of instruction in this area is critical to the clinical implementation of EBP.

Spectral analyses included averages and standard deviations of Cepstral Peak Prominence (CPP), Cepstral Peak Prominence Standard Deviation (CPP SD), Low/High Spectral Ratio (L/H Ratio), Low/High Spectral Ratio Standard Deviation (L/H Ratio SD), Cepstral/Spectral Index of Dysphonia (CSID), and Mean Cepstral Peak Prominence Fundamental Frequency (Mean CPP F0). Statistical analyses revealed significant differences between the spectral analyses of voice characteristics of individuals with FA and those of normal controls for all measures except for CPP SD. The aim of these analyses was to determine spectral differences evident in vowel productions of individuals with FA using new cepstral-derived measures that characterize the phonatory instability and dis-coordination present in this disorder. Such research may not only help develop early non-invasive indicators of ataxia and track disease progression, but also serve to stimulate research into alleviating the symptoms of this devastating disease.

Show moreThis study surveyed speech-language pathologists (SLPs) working in medical settings in the state of Florida in order to: (a) assess the availability of AAC devices, related materials, and services in acute, sub-acute, long-term care, and outpatient medical facilities, (b) examine barriers and supports in providing AAC services to patients with complex communication needs (CCN) in the aforementioned settings, and (c) determine perceived levels of AAC knowledge of health care practitioners. One of the study's major findings was that 97.59% of SLPs served at least one patient they identified as having CCNs, and 94.1% of respondents indicated that their patients could benefit from increased access to AAC devices and service delivery. A notable finding relating to the need for increased AAC-related communication partner instruction (CPI) is as follows: 97% and 100% of respondents indicated that increased CPI for medical practitioners/staff and family members, respectively, were important elements in order to ensure functional communication for individuals with AAC needs in the medical setting. Major barriers to providing AAC services related to device access (i.e., lack of AAC supports / devices, lack of funding for equipment, length of time of device funding). Other barriers were related to the nature of medical settings (i.e., frequently changing caseloads, limited time with patients) and demands of the job (i.e., lack of time to prepare AAC materials / devices). Supports to providing AAC services included low-tech AAC options and mobile technologies. In terms of practitioner knowledge, 57.6% of respondents rated themselves not at all or somewhat knowledgeable regarding AAC. Physicians, nurses, and other rehabilitation professionals were rated as less than knowledgeable by 95%, 97%, and 84.3% of participants, respectively. Overall, the findings of this study suggests there is a high prevalence of patients in medical settings with AAC needs, and some face unmet communication needs resulting from barriers related to the setting itself, lack of access to AAC devices and materials, and limited time spent on AAC service delivery.

Show moreThe purpose of this study was to conduct perceptual analyses, using the initial two-second portions, of prolonged vowels /a/, /i/, and /o/. Two groups of adolescents and young adults were analyzed- one group consisting of 20 individuals with Friedreich's ataxia who were compared to 20 individuals with normal voice (control participants). A trained group of 10 graduate students listened to 132 vowel samples (3 vowels X 40 participants, + 12 samples (10%) for reliability purposes) for a total of 132 perceptual judgments. The students listened to the samples which were randomized onto Dell computers (Optiplex 755) and played through headphones that were set at a comfortable level by the listeners prior to analyzing the voice samples. Listeners used a modified version of the Consensus-Auditory Perceptual Evaluation of Voice (CAPE-VM) to rate the vocal qualities of 'roughness', 'breathiness', and 'strain' in the samples on a 100 millimeter visual analog scale with 0 representing a perception of no roughness, breathiness, or strain, and 100 indicating the most extreme amount of variance from normal voice quality. Statistical analyses were conducted to determine if perceptual measures were significantly different between the two groups. Values on these analyses were expected to be larger for individuals with Friedreich's ataxia than those with normal voice. Results revealed that all three measures were significantly different between the two groups, with those in the Friedreich's ataxia group reported as having increased rough, breathy, and strained components in their voice quality as compared to normal voiced peers. Findings support perceptual measures as useful indicators for reporting changes in the phonatory system due to Friedreich's ataxia.

Show moreThe respiratory tract, in other words, the airway, is the primary airflow path for several physiological activities such as coughing, breathing, and sneezing. Diseases can impact airway functionality through various means including cancer of the head and neck, Neurological disorders such as Parkinson's disease, and sleep disorders and all of which are considered in this study. In this dissertation, numerical modeling techniques were used to simulate three distinct airway diseases: a weak cough leading to aspiration, upper airway patency in obstructive sleep apnea, and tongue cancer in swallow disorders. The work described in this dissertation, therefore, divided into three biomechanical models, of which fluid and particulate dynamics model of cough is the first. Cough is an airway protective mechanism, which results from a coordinated series of respiratory, laryngeal, and pharyngeal muscle activity. Patients with diminished upper airway protection often exhibit cough impairment resulting in aspiration pneumonia. Computational Fluid Dynamics (CFD) technique was used to simulate airflow and penetrant behavior in the airway geometry reconstructed from Computed Tomography (CT) images acquired from participants. The second study describes Obstructive Sleep Apnea (OSA) and the effects of dilator muscular activation on the human retro-lingual airway in OSA. Computations were performed for the inspiration stage of the breathing cycle, utilizing a fluid-structure interaction (FSI) method to couple structural deformation with airflow dynamics. The spatiotemporal deformation of the structures surrounding the airway wall was predicted and found to be in general agreement with observed changes in luminal opening and the distribution of airflow from upright to supine posture. The third study describes the effects of cancer of the tongue base on tongue motion during swallow. A three-dimensional biomechanical model was developed and used to calculate the spatiotemporal deformation of the tongue under a sequence of movements which simulate the oral stage of swallow.
