Show moreCommunity Health Centers (CHCs), designed to provide accessible and affordable health care services to low-income families, were first funded by the Federal Government as part of the War on Poverty in the mid-1960s. Improving healthcare organizational performance efficiency is paramount. It is an especially pressing need for CHCs' because they carry a disproportionate burden of caring for the uninsured within limited budgets. Prior studies suffer from conceptual and methodological limitations. A longitudinal multivariate analysis of factors influencing the performance of CHCs is needed. The purpose of this study is to benchmark CHC performance in terms of technical and cost efficiency, and examine factors that affect its variation. A theoretically grounded non-experimental study design is used, with five waves of panel data from 493 CHCs for the years 2000 through 2004. This study found that data mining and predictor tree analysis of factors influencing the variation in CHCs' technical and cost efficiency yielded inconsistent results. A declining trend in technical efficiency scores over the five-year study period was observed. Based on growth curve modeling, the three factors that influenced technical efficiency at the initial period of the study are: the percentages of Medicare, Medicaid, and Hispanic population being served by the CHCs. The five factors that positively influenced the variation in cost efficiency at the initial period were: the initial score of technical efficiency, the percentage of Hispanic patient population, staffing mix (ratio of providers to total staff), pay mix (ratio of federal grant dollars to total revenue), and percentage of Medicare-eligible. The initial cost-efficiency score and the initial technical efficiency score are negatively associated with the growth trend of technical efficiency. The initial level of technical efficiency is not statistically significantly associated with the growth trend of cost efficiency. The two factors influencing the growth trend of cost efficiency are the growth trend of technical efficiency (with a positive influence) and the initial level of cost efficiency (with a negative influence). Analysis of the effects of contextual and organizational-structural variables on the technical efficiency and cost efficiency of community health centers found that the explanatory power of the predictors is much greater for cost efficiency than for technical efficiency. The study lends support to contingency theory and confirms the independent and additive influences of contextual and organizational predictors on efficiency. Irrespective of the efficiency measures, contextual factors have much more influence on CHCs' efficiency than design (organizational structural) factors do. The three study hypotheses supported by multivariate analysis are: technical efficiency is associated with contextual factors and organizational factors; cost efficiency is associated with contextual factors and organizational factors; and technical efficiency positively affects cost efficiency.

Show moreThe United States currently provides a health care system that is neither efficient nor equitable. Despite outspending the world on health care, over three-fourths of developed countries produce better health outcomes (Auerbach et al., 2000). Simultaneously, the "Ecological School of Thought" has documented the large impact that social, economic, and environmental circumstances play in health outcomes. Unfortunately, these 'ecological" studies are frequently conducted without theoretical justification, and rely solely on a cross-sectional research design and a myriad of unrelated variables. This study represents an important step towards the development of a true theory of "ecology". More specifically, we argue that the adversity associated with socio-economic disadvantage, social disorganization, and a lack of health care resources, leads to adverse health outcomes, represented by sentinel health events. This research employs both a cross-sectional (2000) and longitudinal designs (1990 2000) to assess the antecedents of sentinel health events in 309 United States counties. Structural Equation Modeling was the statistical technique employed in the study. Findings revealed that socioeconomic disadvantage remains a primary contributor to sentinel health. Indeed the economic growth between 1990 and 2000 was associated with increased rates of sentinel health events. Social disorganization was identified as a primary contributor to sentinel health events at a specific time point (2000), but was not significant over time (1990 -2000). Conversely, the inadequacy of health care resources was non-significant in the cross-sectional model (2000), but significant in the longitudinal model (1990 -2000). In both models, racial characteristics were fundamentally linked to ecological predictors of health We found support for the notion that sentinel health events would be reduced through economic equity and the development of healthy environments where community ties are reinforced. Less support is found for saturating given geographical areas with health care resources in order to reduce sentinel health events. Future research should be directed by the theoretical advancements made by this study. More specifically, future studies should examine independent cross-level effects, that is, through the inclusion of behavior variables as mediating factors for ecological constructs.

The highest frequency of juvenile crime, a major social problem, occurs during the 2-4 hours following the end of the school day. Research has demonstrated the benefits of engaging adolecents in prosocial leisure activity. However, no research has studied the issues of free time and leisure education with a delinquent population. This paper documents the impact of a leisure education curriculum on a population of delinquent youth in a randomized experiment. The delinquent youths who received the leisure education reported higher intrinsic motivation and better use of free time. The delinquent youths also reported improved decision making related to their involvement in healthy, prosocial free time activities. Finally, the improvement in the delinquent youths' motivation influenced a significant decrease in the delinquent youths' proneness to boredom during their free time.

Show moreTo date, police organizations have not been rigorously analyzed by organizational scholars and most analysis of these organizations has been captured through a single construct. The purpose of this study is to develop confirmatory police organizational analysis by validating a multi-dimensional conceptual framework that explains the relationships among three constructs: environmental constraints, the design structures of police organizations, and organizational performance indicators. The modeling is deeply rooted in contingency theory, and the influence of isomorphism and institutional theory on the covariance structure model are investigated. One hundred and thirteen local police organizations from the State of Florida are included in this non-experimental, cross-sectional study to determine the direct effect of the environmental constraints on the performance of police organizations, the indirect effect of environmental constraints on the performance of police organizations via the organizational design structure of police organizations, and the direct affect of organizational design structure on performance of police organizations. For the first time, structural equation modeling and data envelopment analysis are used together to confirm the effects of the environment on police organization structure and performance. The results indicate that environmental social economic disparity indicators have a large positive effect on police resources and a medium effect on police efficiency. Propensity of crime indicators has a large negative effect on police resources, and population density has a small to medium negative effect on crime clearance. Structure has a much smaller effect on performance than the environment. The results of the efficiency analysis revealed unexpected findings. Three of the top five largest police organizations in the study scored maximum efficiency. The cause of this unexpected result is explained and confirmed in the covariance model. The study methodology and results enhances the understanding of the relationship among the constructs while subjecting environmental and police organizational data to two comprehensive analytical techniques. The policy implications and practical contributions of the study provide new knowledge and information to organizational management of police organizations. Furthermore, the study establishes a new approach to police organizational analysis and police services management research called Police Services Management Research (PSMR) that encompasses a variety of disciplines with a primary responsibility of theory building and the selection of theoretical framework.

Of these patients, 400,000-500,000 will die annually giving the ICU the distinction of having the highest rate of mortality and complications in the hospital setting. Studies have demonstrated that one in ten patients who die each day in ICUs might survive if intensivists were present to manage clinical care and direct treatment plans (Randolph & Pronovost, 2002; Dimick, Pronovost, Heitmiller & Lipsett, 2001; Pronovost et al., 2002). The utilization of supplemental remote telemedicine has been investigated as a means of compensating for the limited resource of intensivists (Breslow et al., 2004; Rosenfeld et al., 2000). One specific use of this technology, the electronic intensive care unit or eICU®, has previously demonstrated the potential to improve physiological and economic outcomes in ICU patients through the use of integrated decision-support and patient data systems. The present study focuses on the eICU® as a 21st century technology capable of improving the quality of patient care and identifies those factors most likely to impact the success of this clinical intervention. This research utilizes a non-experimental pre-and post-intervention study design and examines patient data collected on all admissions to five ICUs managed by two regional tertiary care hospitals during a 36-month time period. Each ICU is equipped with eICU® software systems that allow intensivist surveillance of patients from a remote facility. The data is analyzed using both structural equation modeling and decision tree regression modeling to statistically evaluate the effects of patient, hospital and unit characteristics on proximal and distal outcomes in ICU patients. As the development of clinical complications subsequently affects patient length of stay, cost of stay, and mortality, it becomes increasingly imperative to seek interventions capable of reducing the risk of unfavorable patient outcomes. This study closely examines one such intervention, the eICU®.

The purpose of this study was to identify policy changes at the organizational, state, and federal levels that had the propensity to impact student success (as measured by student's final letter grade) at FLVS. In addition, this study identified which type of institutional isomporphic policy (coercive, mimetic, or normative) best classified major policy changes in the organization from 1997-2007. The use of institutional theory as the guiding framework for this study proved to be beneficial and enabled the researcher to conclude which types of policy are the most effective in increasing student success in the secondary online education environment. This study utilized ANOVA and regression analysis to detect whether or not changes in policy at the organizational and federal level have a statistically significant impact on student success in the secondary online education environment. This study reveals that student success at FLVS is consistently decreasing and that the change is statistically significant. Regression analysis found that the policy changes at FLVS in this study explain some of the variance detected in the change in the mean, or GPA, of the school. This study found that both coercive and mimetic policies have a statistically significant impact on student success in the secondary online education environment as identified in the isomorphic mechanisms outlined in institutional theory. This study is important to the field of literature regarding secondary online education in that it opens the discussion regarding types of policy and the potential impact that policy changes have on student success in the secondary online education environment. In addition, this study serves as a framework upon which future studies can be conducted and are recommended in this study.

Show moreThe purpose of this research was to evaluate the causal relationships between job strain, the practice environment and the use of coping skills in order to assist in the prediction of nurses who are at risk for voluntary turnover. It was conducted at the level of the individual nurse employee in order to better understand the health consequences associated with job strain, the factors in the professional practice environment which may contribute to the propensity to leave and the influence of coping behaviors in response to workplace stressors. It was undertaken with the intention of identifying intervention strategies which will promote a healthy workforce and the retention of nurses in the workplace. An exploratory cross-sectional survey of 1235 staff nurses employed on the intensive, progressive and general medical-surgical nursing units of seven hospitals associated with a major Central Florida healthcare network tested a client-centered model in an effort to identify nurses vulnerable to the health consequences of job strain using structural equation modeling. Human subject protection was assured. An 82 item questionnaire was used to collect demographic data and measure responses to items associated with the constructs of health status, autonomy, collaboration, decentralization, coping, satisfaction, absenteeism and intent to leave. A variety instruments that were previously demonstrated as valid and reliable were used in the construction of the instrument. Subjects were also given the option of including additional written comments. A total of 325 surveys were returned, of which 308 met inclusion criteria, for a response rate of 25%. Data analysis determined that the measurement of job strain as a function of self-assessed generic health status was predictive of propensity to leave (ã = -.21). The experience of job strain shared a strong association with indicators of mental health status. Job strain was significantly influenced by coping behavior (ã = .56) which targeted activities associated with sustaining and balancing. Anecdotal remarks suggested that the need for balance influenced perceptions regarding stressors in the workplace. The professional practice environment was associated negatively with the propensity to leave (ã = -.58). Those staff nurses who experienced higher levels of autonomy expressed a greater degree of satisfaction and lower intent to leave. The variables of collaboration and decentralization contributed minimally to the construct of professional practice. Anecdotal remarks suggested that the low contribution of collaboration and decentralization contributed to a sense of powerlessness and frustration with work related circumstances. The influence of job strain, coping and the professional practice environment upon staff nurses suggests that health promotion strategies, efforts to enhance coping behavior and promotion of a professional practice environment will increase employee satisfaction and reduce intent to leave. Adoption of policies and procedures which support the health and well-being of individual staff members will benefit employees, strengthen the organizations in which they practice and promote the overall retention of nurses in the face of looming nurse shortages.

Show moreResearch has shown that female physicians continue to earn less than their male counterparts. From both social justice and feminist perspectives, laws requiring equal pay should provide just income for females as compared to males. However, the literature continues to indicate that in general females earn less than males, a trend that is also true for physicians. Theoretically informed postulates are measured here with structural equation modeling to test the influence of the unique latent construct "specialization" on the income gap while controlling for demographic and contextual variables. The analysis tests the assumption that the influence of specialization is the same for females and males. If the influence of specialization and other variables differs by gender, gender bias in physician income may be conceptually implied. The study uses three waves of data from the Community Tracking Study Physician Survey (CTS). The study finds an income gap between females and males in three waves of the CTS. Gini coefficients show females continue to experience greater income inequality than males, with the Lorenz curves for males being closer to the equality lines. Using 1999 income data, there is a statistically significant income gap between female and male physicians when controlling for weeks worked. Information Technology (IT) use was found to be the most reliable construct measuring the unique latent variable specialization. Structural equation modeling showed indicators of specialization have an influence on the income gap. The variables in the CTS Physician Survey made for a poor construct that failed to measure specialization as a uni-dimensional construct. The variables that influence the income gap were different for females than for males. As policy makers revise or create better laws to protect income equality, gender differences must be taken into consideration.

Show moreABSTRACT This research evaluates the relationships of time balance, social relations, role conflict, perception of work environment, and fourteen control variables to police officers' well-being in Turkish National Police. Well-being is identified in the management literature as having a strong relationship with performance. Therefore, by finding the factors affecting well-being, this research seeks to identify intervention strategies, which can promote a healthy workforce and police performance. Such interventions, in addition, may improve police performance through improved well-being. Individual police officers were analyzed to better understand the relationship between work environment on family life, social life, and the well-being of the police officers. A cross-sectional survey was conducted in the seven geographic regions of Turkey for all branches of Turkish National Police. Structural equation modeling (SEM) was used to validate the measurement of latent constructs and their relationships. A 45-item questionnaire collected demographic data and items associated with the latent constructs such as time balance, social relations, role conflict, perception of work environment, and the police officers' well-being. This 45-item questionnaire was based on two survey instruments that have been used by Eurofound in Europe for two decades. The response rate for the questionnaire in this dissertation was 47.14% with 495 respondents out of 1,050 subjects. The analysis revealed statistically significant relationships between following latent constructs: time balance and well-being (an indirect effect via role conflict), time balance and social relations, time balance and role conflict, social relations and role conflict, role conflict and well-being, and perception of work environment and well-being. In addition, six control variables (rank, department, optimism, isolation, income sufficiency, and working days per week) were statistically significantly related with well-being. No direct significant relationship was found between time balance and well-being, and social relations and well-being constructs. Eight control variables (gender, marital status, service time, extra work, confusion, region, work type, and working hours per day) had no significant relationship with well-being. These findings support some commonly expressed complaints of police officers. These findings also suggest that attention should be paid to the effects of time balance, income sufficiency, work environment, and workdays on the well-being of the officers.

Show moreThe purpose of this research was to examine the influence of police organizational culture on officers' perception of integrity in the Turkish National Police (TNP). Adopting an organizational rather than an individualistic perspective, this study focused on police organizational culture to investigate the causes of integrity problems in the TNP. While focusing on police organizational culture, this study also aimed to determine to what extent officers' perceptions of integrity are influenced by personal and occupational attributes such as age, gender, rank, income, educational level, years of employment, and assignment type. The research was conducted in Turkey's two largest cities, Istanbul and Ankara, as well as the largest city of southeastern Turkey, Diyarbakir. Three hundred officers, consisting of 200 regular police officers and 100 ranked officers, were selected from each city, making a sample size of 900 in total. With a total of 507 respondents, after eliminating cases with missing values, the study achieved a 57% response rate. Structural equation modeling was used to test the hypothesized relationships, since it is the most appropriate statistical method for testing hypotheses based on relations among latent and observed variables. This method enabled the researcher to measure the perception of police integrity and the perception of police organizational culture with their multiple indicators, providing rigorous measurements for both constructs. The results of the statistical analysis supported the research hypothesis. The findings showed that officers' perception of police organizational culture negatively influenced their perception of integrity. It was also found that officers' age and educational level also had a statistically significant influence on their perception of integrity.

Show moreHuman related accidents in high-risk industries amount to a significant economic hazard and incur tremendous damages, causing excessive operational costs and loss of life. The aviation industry now observes human-related accidents more frequently than in the past, an upswing attributable to cutting-edge technology usage and the complex systems employed by aviation organizations. Historically, aviation accidents have been attributed to individual unsafe behavior. However, contemporary accident causation models suggest that organizational-level factors influence individual safety performance, as human-related accidents take place in an organizational context. The present study examines the formation of organizational safety culture and influence on individualsÃÂ' safety behavior in a police aviation environment. The theory of planned behavior guides the study model in explaining individual variability in safety behavior via organizational safety culture. The study conceptualized organizational safety culture and individual safety behavior as multidimensional constructs. Confirmatory factor analysis was conducted for each latent construct to validate the construct validity for each measurement model. Organizational safety culture was observed via safety climate facets, which contained four subcomponents including individual attitude, group norms, management attitude, and workplace pressures. Individual safety behavior contained violation and error components observed by self-reported statements. Structural equation modeling was conducted to test the study hypotheses. Utilizing a sample of 210 employees from the Turkish National Police Aviation Department, a 53-item survey was conducted to measure individualsÃÂ' safety culture perceptions and self-reported safety behaviors. The results suggest that individual safety behavior is significantly influenced by organizational safety culture. Except for the relation between workplace pressures and intention, all suggested relations and correlations were statistically significant. The four-factor measurement model of organizational safety climate fit reasonably well to the data, and most correlations between the safety climate components were significant at the .05 level. IndividualsÃÂ' self-reported error behavior is positively associated with age, and individualsÃÂ' self-reported violation behavior is positively associated with years of service. Overall, along with organizational safety culture, age and service-year variables accounted for 65% of the variance in intention, 55% of the variance in violation behavior, and 68% of the variance in error behavior. Lastly, no significant difference manifested among pilots, maintenance personnel, and office staff according to their self-related safety behaviors. The findings have theoretical, policy, and managerial implications. First, the theory of planned behavior was tested, and its usefulness in explaining individualsÃÂ' safety behavior was demonstrated. The survey instrument of the study, and multi-dimensional measurement models for organizational safety climate and individual safety behavior were theoretical contributions of the study. Second, the emergence of informal organizational structures and their effects on individuals indicated several policy implications. The study also revealed the importance of informal structures in organizations performing in high-risk environments, especially in designing safety systems, safety policies, and regulations. Policy modification was suggested to overcome anticipated obstacles and the perceived difficulty of working with safety procedures. The influences of age on error behavior and years of service on violation behavior point to the need for several policy modifications regarding task assignment, personnel recruitment, health reports, and violation assessment policies. As well, managerial implications were suggested, including changing individualsÃÂ' perceptions of management and group attitudes toward safety. The negative influence of anticipated obstacles and the perceived difficulties of safety procedures on individual safety behavior pointed out managementÃÂ's role in reducing risks and accidents by designing intervention programs to improve safety performance, and formulating proactive solutions for problems typically leading to accidents and injuries.

Show morePurpose: The purpose of this study is to understand the relationships among structural complexity, process adequacy, and level of quality in a hospital environment of care from the perspective of the biomedical engineering technician (BMET). The BMET health support occupation is under-represented in research. The BMET influences on patient outcomes were observed indirectly through the influence on nursing performance. The unit of analysis is biomedical engineering technician. The study examined the predictors of adverse events in hospital care and suggested how to promote reduction in adverse events (hospital acquired infections) associated with medical equipment. Methods: Primary data were gathered by the Biomedical Engineering Interdepartmental Survey of BMETs' professional perception of organizational factors (Structural Complexity), process factors (Process Adequacy), Level of Quality and control variables that characterizes the study population. A total of 317 BMETs in 46 states and the District of Columbia completed the survey. The survey instrument was developed using Dillman's Tailored Design Methods and validated for reliability. A majority of respondents reported 5+ years of experience working at large, non-profit, Joint Commission accredited, urban facilities across 5 regions. The effect of structural complexity and process adequacy was analyzed by structural equation modeling (SEM) under the theoretical framework of Donabedian's Structure-Process-Outcome model. Findings: The SEM data analysis indicates strong, positive relationships between constructs as statistically significant (2-tailed) with normal distribution: 1) Structural Complexity and Process Adequacy at Beta =.889, t=7.248, p<0.001; 2) Process Adequacy and Level of Quality at Beta =.563, t=3.136, p=0.002; and 3) Structural Complexity and Level of Quality at Beta =.430, t=2.469, and p=0.014. Translation of these findings into equation form as follows: Level of Quality = .889 Structural Complexity + .563 Process Adequacy The study finds several determinants of quality derived from structural complexity including 1) uniform standards, 2) inter-professional training, and 3) coordination evidence. In addition, the intervening effect of process adequacy comprising regular meetings, equipment purchasing involvement, formal equipment training across departments, and formal department information on the level of quality is supported. Conclusion: Predictors identified from interdepartmental and inter-professional partnerships and associated processes suggest that integration of the biomedical engineering technician into the hospital delivery system can improve the quality of care. Administrators can manage and improve quality through employing simple, effective and efficient solutions such as 1) updating internal hospital policy to require regularly scheduled meetings between nursing and biomedical staff regarding equipment issues, 2) linking the BMET department goals to organization objectives, 3) interdepartmental reporting of hospital acquired infections, and 4) standardizing clinical engineering practices to facilitate increased internal and external hospital quality.

Show moreThe New Great Game is a geopolitical competition between regional stakeholders over energy resources in Central Asia. The author seeks to use the expected utility voting model based on Black's median voter theorem for forecasting the New Great Game in Central Asia. To judge the external validity of the voting model, the author uses data from the Correlates of War project data set, to formulate three distinct models based only on the numbers in 1992 and 1993. Capabilities and alliance data were used to develop balance of power positions and compare the outcome of 100 simulations to the actual outcome in 2000 based on Correlates of War project data. This allows us to judge whether the emergence of Russia's weak advantage as well as the continuation of the competition in the New Great Game as of 2000 could have been predicted based on what was known in 1992 and 1993. By using only one year's data to forecast the New Great Game, we are able to eliminate historical and researcher bias and judge the applicability of the model in global policy and strategic analysis.

History showed that well-implemented crisis management policies can save lives and reduce costs in a disaster. Literature offered various suggestions for more effective crisis management policies with different techniques utilizing different theoretical frameworks. Informal relationships among crisis management employees were suggested to have a positive impact on crisis management effectiveness. Yet it was not demonstrated with advanced statistical tools if there is such a relationship. This study considers crisis management effort as a network effort and employs complex adaptive systems theory in order to understand factors influencing effectiveness of crisis management networks. Complex adaptive systems theory presents that more open communication lines in a given network or an organization would increase effectiveness of it since inner processes of the network or organization would obtain more information from the chaotic environment. Quality of informal relationships (casual relationships, social capital etc.) was hypothesized as a tool to open more communication lines within an agency which would eventually increase effectiveness of the network constructed by the organization. Based on the theoretical framework, adaptiveness capacity of the agencies was also tested in order to understand a correlation between adaptation and effectiveness of crisis management networks. Multiple case-study method was employed to identify incidents that can represent crisis management in full perspective. Terrorist attacks carried upon by the same terrorist network hit New York in 2001, Istanbul in 2003, Madrid in 2004, and London in 2005 were selected. First response phase of crisis management and policy changes after and before the attacks were discussed. Public administration processes and other social-economical conditions of countries were examined in terms of crisis management structure. Names of key agencies of selected crisis management systems were suggested by a social network analysis tool-UCINET. Six key agencies per incident were targeted for surveys. Surveys included a nine-item-quality of informal relationships, four-item-adaptiveness capability, and ten-item-perceived effectiveness of crisis management networks-scales. Respondents were asked to fill in online surveys where they could refer to their colleagues in the same incidents. 230 respondents were aimed and 246 survey responses were obtained as a result. Surveys formed a structural equation model representing 23 observed factors and 2 latent constructs. Confirmatory factor analysis was utilized to validate hypothesis-driven conceptual models. Quality of informal relationships was found to have a significant positive impact on perceived crisis management network effectiveness (Standardized regression coefficient = .39). Two of the adaptiveness variables, openness to change and intra-organizational training were also positively correlated with the dependent variable of the study (Standardized regression coefficient = .40 and .26 respectively). Turkish and American groups' differences suggested a social-economical difference in societies. Majority of the respondents were some type of managers which made it possible to generalize the results for all phases of crisis management. Discussions suggested improved informal relationships among crisis management employees to provide a better crisis management during an extreme event. Collaborative social events were offered to improve crisis management effectiveness. An agency's openness to change proposed that a crisis management organization should be flexible in rules and structureto gain more efficacy. The other adaptiveness variable, intra-organizational training efforts were proposed to have certain influence on effectiveness of crisis management network. Factors built latent construct of perceived crisis management effectiveness were also found out to be important on crisis management, which of some are ability to carry out generic crisis management functions, mobilize personnel and resources efficiently, process information adequately, blend emergent and established entities, provide appropriate reports for news media etc. Study contributed to the complex adaptive system theory since the fundamentals of the theory were tested with an advanced quantitative method. Non-linear relationships within a system were tested in order to reveal a correlation as the theory suggested, where the results were convincingly positive. Crisis management networks' effectiveness was demonstrated to be validated by a ten-item-scale successfully. Future research might utilize more disaster cases both natural and manmade, search for impact of different communication tools within a system, and look at the relationships among members of crisis management networks instead looking within an organization.

According to research, patients find value in having access to information contained in their medical records. Often a glossary is required to aid in interpreting the information and understanding the content. However, giving patients the ability to speak with providers about their medical conditions empowers them to participate as informed healthcare consumers. The majority of patients (75%) at Medical Specialists expressed their intention to adopt the PHR if it is made available to them. Although the perceived usefulness of a PHR was a significant determining factor, comfort level with technology, health literacy, and socioeconomic status were indirectly related to intention to adopt as well. Perceived health status was not found to be a significant factor in this population for determining intention to adopt a PHR. The majority of patients in each category of gender, age, marital status, and race/ethnicity (except American Indian/Alaska Native) expressed interest in adopting a PHR, with most categories being above 70%. Findings indicate a broad acceptance of this new technology by the patients of Medical Specialists. Improvement of adoption and use rates may depend on availability of office staff for hands-on training as well as assistance with interpretation of medical information. Hopefully, over time technology barriers will disappear, and usefulness of the information will promote increased demand.

The aim of this study is to evaluate the impact of social capital on youth behavior and to suggest evidence-based policy interventions. Social capital refers to individual embeddedness in web of social relations and their behaviors guided by social structure. Therefore, adolescents' social interactions with their peers, parents, and community were investigated. The substance use was measured by the usage of cigarettes, alcohol, marijuana, and inhalants in the past year. The type of activities adolescents participate in, the time and type of intra-familial interactions between parents and adolescents, and the type of peer groups adolescents interact with were employed as indicators of social capital. In other words, this study focuses on the relationship between youth substance use and the impact of parents, peers, and youth activities. Moreover, the study examined not only the correlation between social capital and substance use, but also the variation in substance use among youth by age, gender, ethnicity, income level, and mobility. The data, National Survey on Drug Use and Health (2005, 2006, and 2007), was collected by the United States Department of Health and Human Service, Substance Abuse and Mental Health Services Administration Office of Applied Studies. The sample size for each year was around 17.000. Structural Equation Modeling (SEM) was used to test the hypothesized. The results of the statistical analysis supported the research hypothesis.Findings show that there is a relationship between youth substance use and social capital. All three dimensions of social capital (peer impact, family attachments, and youth activities) were found to be statistically significant. While peer influence is positively correlated with substance use, family attachment and youth activities have a negative relationship with substance use. The impact of social capital however varies by age, gender, ethnicity, mobility, and income level. The study also contributes to the social capital literature by integrating different perspectives in social capital and substance use literature. Moreover, it successfully demonstrates how social capital can be utilized as a policy and intervention tool.

Show moreAs in many countries, burglary is very important crime in Turkey. Scientific methods and techniques are needed to solve complex burglary cases. This study was completed in the Bursa Police context since they classified many crime data conducive to scientific studies under a project called BEMTAP. The main purpose of this study is to examine the factors influencing the commission of burglaries, using an epid-criminological perspective. While measuring the relative importance of predictor variables on the number of burglaries and on repeated burglaries, two different models were constructed and validated. For the first model of predictors, Hierarchical Multiple Regression Analysis were performed; for the second one, logistic regression was used. The results show that offender factors are more influential than opportunity and environmental factors in explaining the variability in frequency of burglaries committed and the likelihood to commit repeated burglaries. In conclusion, the best way to reduce burglary rate is to focus on offender factors. Dealing with oppportunity and environmental factors would also contribute to a decreased burglary rate.

However, different vehicles of delivering care (e.g., hospitals, health clinics, etc.) have differing organizational capacity that may or may not enable them to overcome the challenges of expanded provision. Consequently, this research employed structural contingency and organizational performance models to investigate the impact of organizational factors on productivity growth, while recognizing that contextual factors also affect the delivery of care. Latent growth curve modeling was used to study a national panel of 708 U.S. hospital-based Rural Health Clinics for the years 2005 to 2008. Productivity was measured through dynamic slacks-based data envelopment analyses. Unconditional and conditional linear growth curve models were fitted to data. Findings revealed that 1) hospital-based clinics with higher baseline levels of productivity in 2005 had a slower rate of growth in productivity for the years 2006 to 2008, 2) hospital-based clinics with physicians had significantly higher productivity, 3) hospital-based clinics in urban focused areas had significantly higher productivity, 4) newer hospital-based clinics had significantly higher productivity, and 5) prospective payment system was negatively related to the rate of change in productivity growth. Organizational and contextual factors included in this study significantly explained initial differences in productivity but were unable to explain productivity growth.Future research could improve the study by 1) including additional explanatory variables, such as the use of technology and disease management programs, 2) adjusting productivity measures by case mix measures, and 3) conducting truncated panel data regression with Monte Carlo simulation.

There is much evidence that income inequality is one of these root causes but research suggesting the effect of income inequality is mediated by social capital has complicated the relationship, as have theories of causality that take different approaches. This study takes an ecological approach to these issues to test the relationship between income inequality, social capital and selected adverse outcomes proposed by the relative income hypothesis. The relative income hypothesis posits that the impact of income inequality on adverse outcomes is mediated by social capital. The study used a retrospective cross-sectional design to analyze county-level data for the year 2000 with a structural equation model composed of three constructs: income inequality, modeled by four common measures; a social capital construct based on a model developed by Rupasingha, Goetz and Freshwater (2006); and an adverse outcomes construct designed as a parsimonious measure of social outcomes in four public affairs disciplinary areas. The test of the path presumed by the relative income hypothesis revealed both a direct effect of income inequality and indirect effect of inequality through social capital. However, the direct effect of income inequality on outcomes was significantly larger than the indirect effect, indicating the relationship is moderated, rather than mediated, by social capital. Since the impact of social capital on the selected adverse outcomes was relatively small, and the final model failed to achieve statistical significance, the relative income hypothesis that income inequality exerts its primary effect on outcomes through social capital was rejected.

These indicators have remained at exceptionally high rates in the United States despite the levels of resources and technology. In the quest to understand that discrepancy, among the ranges of theories and models for explaining poor pregnancy outcomes an emerging concept is coming to attention: social capital. In order to test whether maternal social capital has an impact on pregnancy outcome, women in a Healthy Start program were surveyed over a 13-month period to assess their social capital levels and then their pregnancy outcomes. What emerged was that maternal social capital can predict up to 47% of the variance in pregnancy outcome. That is a powerful research result considering that previously there has been no literature tracing a link between maternal social capital and pregnancy outcome. In this study, maternal risk factors adversely affect up to 30% of the variance in pregnancy outcomes. Previous research has focused on maternal risk factors as the primary reason for high rates of preterm delivery, low birth weight, and infant mortality in the United States. However, this research found that in the sample of women at risk for adverse pregnancy outcomes, maternal risk factors had a very strong influence on maternal social capital (R-square=65%) while their effects on pregnancy outcomes were about half of their effects on social capital. This result suggests that social capital mediates the effects of maternal risk factors on pregnancy outcomes. It appears that one of the reasons that the high rates of adverse pregnancy outcomes in the United States have remained a mystery is that maternal social capital has not been taken into account.
